Comparison of Single-Step Transepithelial Photorefractive Keratectomy With or Without Mitomycin C in Mild to Moderate Myopia

Abstract: Single-step transepithelial PRK with or without MMC showed similar efficacy and incidence of haze in eyes with mild to moderate myopia with total ablation depths of 160 μm or less. However, MMC-treated eyes showed a worse profile of endothelial cell indices. Applications of MMC in this subgroup of patients may be reconsidered. [J Refract Surg. 2018;34(6):400-407.].

“…None of the authors analyzing double-step TPRK reported predictability results. Among the authors who analyzed the single-step TPRK [ 24 , 27 , 28 , 29 , 30 ], they obtained predictable results comparable to those reported in our study. Our spherical equivalent refractive accuracy showed that 90% of eyes were in ±0.50 D and all eyes were in ±1.00 D. We found similar results in refractive astigmatism accuracy, 92% ≤ 0.50 D and 100% ≤ 1.00 D. Clinch et al [ 20 ] found comparable spherical accuracy results at three months follow-up with 74.4% of eyes within ±0.50 D and 93.5% of eyes ±1.00 D. Lee et al [ 21 ] reported worse results with only 56.9% of eyes within ±0.50 D or less.…”

“…Regarding predictability, our results obtained 1.03x + 0.29 (R 2 = 0.93). Overcorrection or undercorrection may occur during TPRK as well as other corneal surface refractive laser surgeries [ 21 , 27 , 28 ]. None of the authors analyzing double-step TPRK reported predictability results.…”

“…[ 15 ] The use of antimetabolites like MMC is widely used to prevent the haze formation because they are easy to prepare and very accessible for the prevention of corneal haze at a low cost. [ 5 , 16 , 17 ] The MMC can induce alterations in the count and morphology endothelial. [ 7 ] Furthermore, there is the possibility of systemic MMC diffusion into the bloodstream.…”

“…The use of mitomycin C demonstrated in previous studies an high and positive impact on the refractive results and in preventing haze formation post PRK [ 51, 52 ] although several reports showed the absence of significant differences in haze formation in patients with or without the use of mitomycin C during the follow‐up. [ 53, 54 ]…”
