Comparison of Survival Between Autologous and Allogeneic Stem Cell Transplantation in Patients with Relapsed or Refractory B-Cell Non-Hodgkin Lymphoma: A Meta-Analysis

Abstract: This study aimed to compare the efficacy of allogeneic stem cell transplantation (allo-SCT) versus autologous SCT (auto-SCT) in patients with relapsed or refractory B-cell non-Hodgkin lymphoma (B-NHL). Medline, CENTRAL, and EMBASE databases through December 31, 2019 were searched. The primary endpoints were overall survival (OS) and progression-free survival (PFS) rates. The secondary outcomes include transplant-related mortality (TRM), event-free survival, relapse/or progression, and nonrelapse mortality (NRM… Show more

“…The most frequently observed adverse events comprised febrile neutropenia, mucositis, and nausea, as reported before elsewhere [ 10 , 15 18 , 32 35 ]. The observed NRM of 4% at day 100 after ASCT was in a similar range as compared to other recent retrospective studies [ 21 , 22 , 26 , 28 ]. Although NRM mostly occurred within 3 years after ASCT, NRM steadily increases over time, with infections representing the leading cause of late NRM events.…”

“…Infections and mucositis represent the most relevant adverse events after ASCT, while other toxicities including impairment of renal and liver function are less prevalent [ 20 , 21 ]. Non-relapse mortality varies between 1 to 4% at day 100 after ASCT [ 21 , 22 ]. Death after ASCT is mostly related to relapse or progression of lymphoma, followed by infections, organ failure, and secondary malignancies [ 23 ].…”

“…Allo-HCT can produce longterm disease control via the graft-versus-lymphoma effect. [29][30][31][32] In the present study, we aimed to evaluate the prognostic value of 18F-FDG PET-CT after allo-SCT in LBL patients using DS. For all 63 LBL patients, positive post-SCT 18F-FDG PET-CT was associated with a lower PFS, but not OS.…”

“…Unfortunately, both MSC selection methods have significant drawbacks: allogeneic transplantation is associated with potential immune rejection, donor-donor heterogeneity, specific immunological memory and quick clearance after infusion, while autologous MSCs are characterised by long-time availability, limited cell dose, donor variability issues and prohibitive cost. However, on balance, despite their own merits and limitations, the allogeneic approach seems to be superior [244][245][246].…”
