Background: non-bismuth sequential therapy (SEQ) was suggested as a first-line anti-Helicobacter pylori treatment alternative to standard triple therapy (STT). Methods: We conducted a systematic review with a meta-analysis of randomized controlled trials (RCTs) comparing the efficacy of 10-day SEQ vs. STT (of at least 7 days) using bibliographical searches up to July 2021, including treatment-naïve adult or children. The intention-to-treat (ITT) eradication rate and the risk difference (RD) were calculated. Results: Overall, 69 RCTs were evaluated, including 19,657 patients (9486 in SEQ; 10,171 in STT). Overall, SEQ was significantly more effective than STT (82% vs. 75%; RD 0.08; p < 0.001). The results were highly heterogeneous (I2 = 68%), and 38 studies did not demonstrate differences between therapies. Subgroup analyses suggested that patients with clarithromycin resistance only and all geographical areas but South America could benefit more from SEQ. Both therapies have evolved over the years, showing similar results when STT lasted 14 days; however, a tendency toward lower SEQ efficacy was noted from 2010 onwards. Conclusions: Prior to 2010, SEQ was significantly more effective than STT, notably when 7-day STT was prescribed. A tendency toward lower differences between SEQ and STT has been noted, especially when using 10-day STT. None of the therapies achieved an optimal efficacy and therefore cannot be recommended as a valid first-line H. pylori treatment.

H. pylori (Helicobacter pylori) is the most prevalent infection worldwide which leads to gastric ulcer and cancer. In our research work, patients infected with H. pylori were given concomitant treatment for 10 and 12 days and compared to find the efficiency of treatment. Objective: The objective of this study is to find a cost-effective therapeutic method for the treatment of H. pylori. Previous 14-day treatment was effective but it was expensive due to the high cost of antibiotics as well as the negative side effects of the treatment, so an alternative method is required to reduce the need of costly antibiotics and the duration of treatment. Study design: It is a comparative study with statistical approach, conducted in the medicine department of Sialkot Medical College, Sialkot for the duration of six months from June 2021 to November 2021. Methods: For this study, 196 patients with gastric ulcers were selected and confirmed H. pylori positive by a diagnostic test. These patients randomly distributed into two groups and given treatment for 10 and 12 days respectively. UBT was used to find either H. pylori is eradicated or not. “Registry of clinical trials” registered this experiment. Results: For this research work, 196 patients were selected, after the concomitant therapy eradication rate was analyzed that comes out as 83 % (CI 95.0%: 76-90) and 88 % (CI 95%: 82-94) for 10 and 12 day therapy, respectively (p = 0.23). Then, prediction of per-protocol eradication was done and it came out as 85% (CI 95%: 79-92) and 92% (CI 95%: 87-97), respectively (p = 0.18). When side effects were compared between two groups, no statistical difference exist between two (3.5% vs. 8.0%; p = 0.419). Conclusion: All the results suggested that the concomitant therapy that was given for 12 days was more effective for treatment and per-protocol assessment. For the reduction of the cost of antibiotics, 12 day treatment is more effective for treatment as well as the replacement of 14 day treatment that was suggested by international rules. Background: Helicobacter pylori, the most prevalent infection in the world, has great importance due to being related to peptic ulcer disease, gastric metaplasia, dysplasia, and even gastric adenocarcinoma or mucosa-associated lymphoid tissue (MALT) lymphoma. The standard H. pylori eradication regimen is based on antibiotic susceptibility testing. If susceptibility testing is not available, a standard treatment regimen will be recommended based on records of H. pylori resistance rates to antibiotics in a region or locally proven highly effective regimens (equal to or higher than 90% eradication rate). The aim of this review was to define suitable recommendations for local treatment in different cities of Iran. Methods: This review article consists of randomized controlled trials related to H. pylori eradication in Iran. Data including the kind of therapy, number of patients and per-protocol H. pylori eradication rates were recorded in data gathering forms. Data search was conducted in PubMed and Google Scholar databases from 2018 to December 2023. Results: According to our review of Iranian articles regarding first-line H. pylori eradication regimens, these treatment protocols could be recommended: Bismuth-clarithromycin quadruple therapy in Ardabil, bismuth-clarithromycin quadruple therapy with probiotics in Birjand, standard triple therapy in Ilam, bismuth quadruple therapy or bismuth triple therapy or concomitant regimen in Sari, sequential therapy in Tehran and bismuth quadruple therapy in Yazd. These regimes can be extended to other regions that have a similar situation. According to the reports of Iranian researchers, a quinolone-containing regimen (levofloxacin preferred) is recommended for second-line eradication therapy. Conclusion: Various H. pylori eradication regimens can be used as first-line therapy; however, choices for second-line therapy are limited. We recommend the quinolone-containing regimens as the preferred second-line therapy.
