Difficult or failed intubation is a major contributor to morbidity for patients and to liability for the provider. Research to improve understanding, prevention, and management of such complications remains an anesthetic priority, and a driving force behind continuous improvements in intubation techniques and intubation equipment. The purpose of this review article is to focus on the video-assisted intubating stylet technique (VS; also known as the Shikani optical stylet technique for intubation) and video-assisted optical stylet devices, both for routine use and alternative rescue application for tracheal intubation, and stress their advantages as compared to conventional direct laryngoscopy and videolaryngoscopy. The VS technique was introduced by Dr. Alan Shikani in 1996 and popularized with the advent of the Shikani optical stylet and subsequent similar stylets variations. We focus on the clinical details of the technique itself, and on the various advantages and troubleshooting under different clinical scenarios and practice settings. In our experience, video-assisted intubating stylet technique often constitutes the most appropriate approach both for daily routine and emergency airway management. Furthermore, we also emphasize the importance of video-assisted intubating stylets in enhancing the practitioner systems response when difficult or failed tracheal intubation is encountered.

Background Video laryngoscopes are widely used for tracheal intubation, particularly in challenging airway scenarios. The McGrath MAC, AIRWAY SCOPE®, and AceScope® are popular video laryngoscopes with different design features. This study aimed to compare the effectiveness and usability of these three devices in novice healthcare providers during simulated tracheal intubation scenarios employing a manikin. Methods Sixty novice healthcare providers, including nurses and pharmacists, were enrolled in this randomized crossover study. Participants performed tracheal intubation using the McGrath MAC, AIRWAY SCOPE, and AceScope in both normal airway and cervical spine immobilization models. Primary outcomes were intubation success rate and time to intubation. Secondary outcomes included user preferences, device ease of use, and the incidence of dental injuries. Results The AIRWAY SCOPE demonstrated the shortest intubation time in both normal airway and cervical spine immobilization models (14.90 ± 1.76 seconds and 23.80 ± 2.43 seconds, respectively), followed by the McGrath MAC and AceScope. All devices exhibited high success rates, and there were no significant differences in perceived difficulty among the three video laryngoscopes. The incidence of dental injuries was comparable among the devices, with one notable exception between the AceScope and McGrath MAC in scenario of tracheal intubation using AceScope in a cervical spine immobilization model. Conclusions The AIRWAY SCOPE was the most efficient video laryngoscope in terms of intubation time, followed by the McGrath MAC and AceScope. However, all devices showed high success rates and no significant differences in perceived difficulty. Further research is needed to validate these findings in clinical settings and investigate the impact of device-specific features on intubation outcomes and dental injury incidence.

The use of video laryngoscopy is growing in patients with anatomical factors suggestive of a difficult airway. This case report describes the successful tracheal intubation of a 54-year-old female patient with limited mouth opening scheduled for third molar extraction under general anesthesia. The Airway scope (AWS) along with a gum-elastic bougie was used to secure the airway after failed direct laryngoscopy and video laryngoscopy using the McGrath MAC with an X-blade. The AWS has a J-shaped structure in which the blade approximates the curvature of the pharynx and larynx. This blade shape makes it easy to match the laryngeal axis with the visual field direction, enabling successful tracheal intubation even for patients with limited mouth opening. A major key to successful video laryngoscopy is to select a video laryngoscope based on the anatomical characteristics of patients with a difficult airway.
