Comparison of the Effects of Esketamine/Propofol and Sufentanil/Propofol on the Incidence of Intraoperative Hypoxemia during Bronchoscopy: Protocol for a Randomized, Prospective, Parallel-Group Trial

Abstract: Background: Propofol, ketamine, and sufentanil are the most commonly used anesthetics during bronchoscopy, alone or in combination, for sedation. Esketamine is an s-enantiomer of ketamine racemate and has both sedative and analgesic effects. Esketamine does not inhibit respiration and maintains hemodynamic stability. This study aims to compare the clinical efficacy of esketamine/propofol with sufentanil/propofol for patients during bronchoscopy. Methods: Patients undergoing bronchoscopy will be randomly assign… Show more

“…Propofol is often used in combination with ketamine and opioids to improve efficacy and patient tolerance. Several studies ( Wang et al, 2019 ; Eberl et al, 2020 ; Huang et al, 2022 ; Xu et al, 2022 ) have compared a single-dose combination of esketamine and propofol with propofol and other types of drugs in terms of side effects and degree of sedation undergoing minor procedures, withesketamine doses ranged from 0.15 to 0.5 mg/kg. Esketamine (0.5 mg/kg) has been proven to be safe and well tolerated in Chinese patients undergoing painless gastroscopy, with no serious adverse events ( Wang et al, 2019 ).…”

“…Due to the increased sympathetic tone, voluntary breathing and airway reflexes are maintained, and esketamine therefore stabilizes breathing. 25 Furthermore, propofol is related to a higher risk of respiratory inhibition in a dose-dependent manner. 26 Hence, the reduced dosage of propofol is another key factor for the lower incidence of hypoxemia.…”

“…It has been adopted in some European countries for decades and has been used in Chinese hospitals in recent years. Besides treating depression, esketamine is applied for clinical sedation and analgesia associated with same-day bidirectional endoscopy ( Long et al, 2022 ), pediatric dental surgery ( Xin et al, 2021 ) and mechanical ventilation in ICU patients ( Song et al, 2022 ) When it is used for bronchoscopy, esketamine relaxes bronchiolar muscles and inhibits bronchial constriction ( Huang et al, 2022 ).…”
