Comparison of the Intracarotid Amobarbital Procedure and Interictal Cerebral 18‐Fluorodeoxyglucose Positron Emission Tomography Scans in Refractory Temporal Lobe Epilepsy

Abstract: The relationship between interictal focal hypometabolism determined by 18-fluorodeoxyglucose positron emission tomography (FDG-PET) scans and memory function with the intracarotid amobarbital procedure (IAP) was evaluated in 23 patients with temporal lobe epilepsy. All patients underwent prolonged EEG/video monitoring. The epileptogenic focus was defined by interictal epileptiform discharges and ictal onsets. All 23 patients had recorded seizures arising exclusively from one temporal lobe. PET showed temporal … Show more

“…In our sample asymmetry ratios of these measures were of high lateralizing value. Moreover, we confirmed the high intercorrelation of these asymmetry ratios [14,31,47]. With respect to the absolute values of the used measures we replicated findings that the hippocampal volume and glucose metabolism are not linearly related in TLE patients [1,41].…”

“…FDG-PET showed temporal lobe hypometabolism ipsilateral to the epileptogenic focus in 86%, whereas IAP showed impaired memory of the hemisphere of seizure onset in 65%. Sixty-five percent of patients with unitemporal PET hypometabolism had ipsilateral memory impairment (14). Ninety-six percent of patients with TLE had focal functional deficits on the epileptogenic side hemisphere on either PET or IAP (15).…”

“…Loring et al (13) reported that a significant correlation was present between Wada memory asymmetries and hippocampal volume asymmetries (r ‫ס‬ 0.78). Salanova et al (14,15) compared PET temporal lobe hypometabolism and IAP failure of TLE patients. 65 and 76% of TLE patients with ipsilateral temporal PET hypometabolism showed impaired memory of the hemisphere of seizure onset in two studies.…”

“…Interictal hypometabolism can be seen in 70-80% of the patients with refractory temporal lobe epilepsy after PET scanning 10 . There is usually a good correlation between this hypometabolism and a failure after the contralateral IAP.…”
