Comparison of the OUTBACK® Elite Reentry Catheter and the Bi-directional Approach after Failed Antegrade Approach for Femoro-popliteal Occlusive Disease

Abstract: Aim: A successful antegrade wire crossing for femoro-popliteal chronic total occlusion (FP-CTO) is still a technical challenge. We attempted to demonstrate the safety and feasibility of the OUTBACK® Elite reentry catheter and the bi-directional approach for failed FP-CTO cases with the antegrade approach. Methods: Endovascular therapy for FP-CTO was performed in 219 lesions from May 2013 to December 2016 at Morinomiya Hospital. We retrospectively analyzed the data of 43 consecutive lesions which underwent endov… Show more

“…Dedicated re-entry devices are extremely helpful tools in those circumstances [ 2 , 10 , 18 , 19 , 20 , 21 ]. The use of re-entry catheters has been shown to avoid extending the length of dissection as well as the procedural time [ 2 , 22 ]. In addition, retrograde recanalizations are increasingly used in chronic total occlusion.…”

“…The initial step of the endovascular process is to cross the lesion using a guidewire, and although passing through the true lumen is the principle, subintimal angioplasty [59,60] is an alternative option. Re-entry devices such as OUT-BACK (Cardinal Health Japan G.K., Tokyo) (Figure 5) are used for navigating guidewire tips through subintimal spaces to distal true lumina [61][62][63][64][65][66][67][68][69].…”
