Comparison of the Outcomes of Microendoscopic Discectomy Versus Full-Endoscopic Discectomy for the Treatment of L4/5 Lumbar Disc Herniation

Abstract: Study Design Retrospective Comparative Study. Objectives To compare the outcomes of microendoscopic discectomy (MED) versus full-endoscopic discectomy (FED) for treating L4/5 lumbar disc herniation (LDH). Methods A retrospective study was performed on patients with L4/5 LDH treated using MED (n = 249) or FED (n = 124). A 16-mm tubular retractor and endoscope was used for MED, while a 4.1-mm working channel endoscope was used for FED. Patient background and operative data were collected. The Oswestry Disability… Show more

“…However, it should be noted that the duration depends on multiple factors, including the expertise of the operating surgeon, whether the surgeon performs a sequestrectomy or discectomy, whether the recurrence is on the same side as the previous surgery or on the opposite side, etc. [ 27 ]. Intraoperative blood loss can be a concern in repeat surgery.…”

“…Following the upper limit of the 95% CI for the mean difference in VAS and ODI scores, our intention-to-treat analysis demonstrated that IELD was considered noninferior to MLD in terms terms of a shorter length of stay and reduced blood loss, consistent with previous different approach study. [37][38][39][40] However, the operation time is longer compared to the MLD procedure. The difference operation time between the 2 procedures were attributed to the distinct nature of both surgical procedures, such as patient-specific factors, equipment used, or the skill and experience of the surgeon.…”

“…[ 1 , 8 ] Although repeat discectomy alone has several advantages (i.e., less invasive, shorter hospital stay, and reduced cost), a subset of up to 25% of these patients may later develop instability warranting fusions. [ 2 , 6 , 9 ] Here, we reviewed the literature over the past 5 years for treating recurrent lumbar discs with repeated diskectomy alone.…”
