Comparison of the Uroprotective Efficacy of Mesna and Hbo Treatments in Cyclophosphamide-Induced Hemorrhagic Cystitis

Abstract: The aim of this research was to compare the protective effects of mesna, hyperbaric oxygenation (HBO), and their combination in cyclophosphamide-induced hemorrhagic cystitis in guinea pigs. Following one dose of i.p. 21.5 mg./kg. mesna administration 20 minutes before i.p. 68.1 mg./kg. cyclophosphamide, 3 additional doses of mesna were given every three hours. A total of 8 HBO exposures, 5 of which were applied prophylactically before cyclophosphamide, were performed at 2.8 ATA for 90 minutes 2 times a day. Al… Show more

“…20 As this treatment has been used successfully in patients with HC after pelvic radiotherapy, 17 --19 it may benefit patients with HC after HSCT. 5,10,11 To date, there have been a few case reports of successful HBO therapy in pediatric 13,15 and adult 12,14,16 patients. HBO showed a cure rate of 78.5% in pediatric patients with BKV-associated HSCT.…”

“…5,10 --16 HBO has been shown effective in the treatment of radiation-induced HC, 17 --19 by promoting fibroblast proliferation and capillary angiogenesis, decreasing edema and facilitating damaged hypoxic urothelium. 11,20 We describe our experience with HBO in patients with BKV-associated HC following allogeneic HSCT.…”

“…In the absence of adequate uroprotection, the incidence of this side effect of CYP therapy varies from 2 to 40% in patients taking low doses of CYP on a long-term basis (4). Mortality has been reported to be 2 to 4% in patients with massive hemorrhage, who were treated with a high dose of intravenous CYP (5). The cause of bladder damage appears to be related to acrolein, a urotoxic metabolite of CYP.…”

“…4 Very recently, the combination of mesna and hyperbaric oxygen was shown to completely eliminate HC in guinea pigs. 5 Amifostine [S-2-(3-aminopropylamino) ethyl phosphorothioic acid], designated WR-2721, is an aminothiol that is now in clinical use as a protective agent against chemotherapy-related cytotoxicities. 6 Although all the mechanisms of cytoprotection have not been fully elucidated, the most significant seems to be that the active metabolite, WR-1065, which is a thiol, acts as an intracellular scavenger of free radicals.…”
