A 13 years old male was referred to nuclear medicine department for evaluation of suspected acute multifocal osteomyelitis. The staphylococcal infection was showed by blood culture. We performed Tc-99m methylendiphosphanate (MDP) bone scan, Tc-99m HMPAO leukocyte scan and Tc-99m (V) dimercaptosuccinic acid (DMSA) scintigraphy. In this case, we were able to compare the bone and soft tissue uptakes of Tc-99m (V) DMSA with Tc-99m MDP and Tc-99m HMPAO leucocytes uptakes; simultaneously. Tc-99m (V) DMSA uptake was similar to Tc-99m MDP and Tc-99m HMPAO leucocytes.

Chemical properties of technetium-99m-DL-homocysteine, a possible tumor-imaging agent

The chemical properties of 99mTc-DL-homocysteine (99mTc-Hcy) showing high accumulation in several experimental tumors were investigated. The form of tumor-tropic 99mTc-Hcy was a polymeric complex which appeared at void volume on Sephadex G-15 by eluting with 5 mM Hcy. This complex changed into smaller complexes of ca. 600 molecular weight in the presence of 150 mM NaCl and 5 mM Hcy, suggesting that 99mTc-Hcy was a complex composed of smaller polymers which are weakly bound together by an ionic bond. The complex showed a negative charge. The Hcy/Tc molar ratio in the complex was approximately 2 and no Sn was detected.

99m Tc labelling of DMSA under moderate reduction conditions (alkaline pH and low concentration of Sn(II) leads to formation of various DMSA complexes. One of them 99m Tc(V)-DMSA is proved to be suitable for tumour imaging in nuclear medicine diagnostics. The aims of our study were to define the optimum terms for high yield of 99m Tc(V)-DMSA complex and kit formulation, which can be conveniently applied for diagnosis in nuclear medicine laboratories. The 99m Tc labelled DMSA complexes were analysed by means of ascending thin layer chromatography (TCL) in three mobile phases -methyl ethyl ketone, n-butanol/acetic acid/water 3/2/3 and 0.154 M NaCl. Appropriate formulae were chosen for kit preparation and applied for preliminary clinical investigations.
