Comparison of two methods of tonsillectomy

Abstract: No consensus exists regarding the best method of tonsillectomy. This report concerns two popular methods: 1. electrocautery excision and 2. dissection/snare followed by point coagulation of bleeding sites. To compare these methods, a prospective, randomized, single-blinded study was conducted in which 28 patients had one tonsil removed by dissection/snare and selective cautery of bleeders and the other removed by the electrocautery. Operative time and blood loss were compared. Patients rated the severity of th… Show more

“…Today bipolar electrodissection method, which was defined in 1993, is also being widely used as well as the conventional dissection method [11]. This method has been shown to shorten operation time because it enables the simultaneous realization of dissection and coagulation [12,13,14,15,16,17,18]. Although there are studies pointing to an opposite direction [19,20] the former have been generally accepted by the medical community.…”

“…It is generally accepted that the ideal method should decrease the operating time, blood loss, post operative haemorrhage and particularly the post operative morbidity. 7 Historically diathermy was not used during tonsillectomy because inflammable gases were used for induction.…”

“…1 Ideally, tonsillectomy should be fast, bloodless, and associated with rapid and uncomplicated recovery. 2 In spite of all the new surgical tools and techniques haemorrhage is still a significant complication during and after tonsillectomy and about 5% patients may face this problem at any time from first 24 hours to 10 days after operation. 3 This study concerns two popular methods:bipolar cautery excision and dissection/snare followed by compression with gauge piece and selective cautery of bleeders.…”
