Comparison of ultrasonography and CT for preoperative nodal assessment of patients with papillary thyroid cancer: diagnostic performance according to primary tumor size

Abstract: Background Indications for computed tomography (CT) in preoperative patients with thyroid cancer are still controversial. Purpose To determine the value of CT and ultrasonography (US) in preoperative lymph node assessment of patients with papillary thyroid carcinoma (PTC) according to primary tumor size. Material and Methods A total 453 patients with surgically proven PTC who underwent US and CT for preoperative evaluation in 2010 at our tertiary referral center were included. The diagnostic sensitivity, speci… Show more

“…Ultrasound is not sensitive to the diagnosis of metastatic LN-prRLN because of the interference from the trachea, esophagus, and surrounding tissues. Yang et al reported that the sensitivity, specificity, and accuracy of ultrasound for cervical lymph node metastasis in PTC patients were 38%, 88%, and 57%, respectively (14). It can be speculated that the diagnostic value of ultrasound for LN-prRLN is lower.…”

“…For example, Yang et al. (18) used a scan delay time of 40 s for thyroid CT. Similarly, the scan delay time for enhanced CT scan was 45 s in Zhao et al.…”

“…The protocol for contrast injection during enhanced CT scan for thyroid varies markedly between previous studies (2,3,1820). Several studies only used one delay time (mostly in the range of 35–60 s) for enhanced CT scan (18–20).…”

“…Previous studies have shown that the estimated sensitivity of US in LNM prediction is low (38-59%) (15)(16)(17). Moreover, US is also limited in assessing central lymph nodes and superior mediastinal lymph nodes (18). FNAB is an invasive approach and its sensitivity in evaluating LNM varies and is specific to the operator (19).…”
