Background Mediastinal cysts are benign lesions that may be seen in adulthood as well as in childhood. Mostly congenital lesions constitute 20 to 32% of lesions located in the mediastinum. The main cystic masses are congenital benign cysts (bronchogenic, esophageal replications, neuroenteric, pericardial and thymic cysts), meningocele, mature cystic teratoma, and lymphangioma. In this study, we aimed to analyze the mediastinal cysts operated in our clinic according to the histopathological type, surgical type, morbidity rates and to contribute to the literature on these rare lesions. Methods The records of patients with mediastinal cysts who were operated in Gazi University Faculty of Medicine Department of Thoracic Surgery, between January 2013 and June 2021, were reviewed retrospectively. Results A total of 32 patients were included the study. Thirteen (40.6%) of the patients were male and 19 (59.4%) were female. The mean age was 45 (range: 12–71). The most common symptom in patients was chest pain with 12 patients. Histopathologically, the most common subtype was thymic cyst. Video-assisted thoracic surgery was applied in 19 patients (59.3%), thoracotomy in 8 patients (25%), and sternotomy in 5 patients (15.63%). There was no mortality. Conclusion In patients with mediastinal cysts, the prognosis after complete excision is excellent and rates of morbidity and mortality associated with surgery are low.

Curr Thorac Surg 2020;5(1): ABSTRACTLymphangiomas are often congenital benign tumors that are formed by abnormal proliferation and development of lymphatic tissues. Although it may be localized in all lymphatic chain regions, intrathoracic localization has been reported rarely. Here we present a case of a 24-year-old non-smoker male patient admitted to our clinic with an incidentally found well-circumscribed radioopacity located in the right upper zone of the mediastinal region detected on routine chest x-ray. The computerized thorax tomography revealed a thin layered, fine-lined lesion with cystic content of 8x6 cm, located at the right paratracheal level. The patient underwent a video-assisted thoracoscopic excision of the cyst, and lymphangioma was confirmed based on the histopathologic examination. Although various alternative treatment methods such as chemotherapy, radiotherapy, sclerosing therapy, and laser therapy are used in the treatment of lymphangioma However, their effects on the treatment vary widely in the reported publications.

“…Bu kistlerin birçoğu, VATS yöntemi ile rahatlıkla ve daha kısa sürede çıkarılabilmekte, bu sayede standart torakotomiye kıyasla, daha az anestezi süresi, daha az postoperatif ağrı, daha kısa hastanede kalış süresi ve daha iyi kozmetik sonuç elde edilebilmektedir (6) . Mediastinal kistlerin cerrahi tedavisinde, torakotomi ile VATS'ı karşılaştıran bir çalışmada; VATS ile ameliyat süresi 87,4 dakika iken, torakotomi ile bu süre 123,6 dakika olarak saptanmış ve aradaki fark, istatistiksel olarak da anlamlı bulunmuştur (7) . Aynı çalışmada bir başka karşılaştırma ise, postoperatif hastanede kalış süresi bakımından yapılmış ve torakotomi sonrası 8,2 gün olan bu süre, VATS sonrası 4,3 gün olarak saptanmış, aradaki fark da istatistiksel olarak anlamlı bulunmuştur.…”

Primary mediastinal cysts are rare, congenital and benign lesions originating from various embryological tissue types. However, it can present with similar clinical and radiological findings at almost every age. We present this case to emphasize that two separate mediastinal cysts originating from different embryological origins in different compartments of the mediastinum are extremely rare and that these cysts can be simultaneously resected by videothoracoscopic surgery.
