varía según los países. Las enfermedades respiratorias suponen una carga significativa en los sistemas sanitarios. Objetivos: Valorar la efectividad de la implementación de la enfermera de práctica avanzada en el cuidado del paciente respiratorio, describir su estado actual e identificar sus líneas de investigación. Material y métodos: Revisión sistemática por medio de búsqueda de artículos originales en inglés y/o español, publicados en los últimos 10 años y realizados por trabajadores de la salud en PubMed, CINAHL y Web of Science, evaluando la calidad metodológica según STROBE, COREQ y CONSORT. Resultados: De las 235 publicaciones, se incluyeron 10 documentos para revisión (60% observacionales, 20% cualitativos, 10% estudios mixtos y 10% ensayos clínicos aleatorizados). Se detectó una beneficiosa reducción del gasto sanitario, un aumento de la satisfacción de los pacientes atendidos y menores tiempos de espera. Las enfermeras de práctica avanzada respiratoria actúan de forma multidisciplinaria y multitarea, sin una regulación específica. Conclusiones: Se llega a la conclusión de que es necesario realizar más investigaciones que aborden la enfermería de práctica avanzada en pacientes respiratorios. Se cree que esto aumentará y mantendrá los beneficios de su implementación.

Primary Healthcare Case Management Nurses and Assistance Provided to Chronic Patients: A Narrative Review

Population aging is causing increases in the numbers of chronic diseases, with the consequent need for changes in health systems to better assist patients with chronic conditions. A narrative review was conducted in this study with the objective of analysing the scientific evidence on the care and assistance provided by Case Management Nurses (CMNs) to chronic patients in primary healthcare. A total of 15 articles published in English, Spanish, and Portuguese were selected in the following databases: PubMed, Embase, Cochrane Library, Scopus, Dialnet, Cinahl, and Web of Science. In total, 46.6% of the studies showed the assistance provided by CMNs for chronic pathologies. Most of the articles selected (80%) considered that the assistance offered by case management nurses in relation to chronic diseases is effective, enabling cost reductions, which supposes benefits at the economic and political levels. It was concluded that CMNs have proven to be efficient in caring for people with chronic diseases, improving the quality of life of these people and their caregivers; therefore, they have a fundamental role in the PHC.

Aim This study assessed the application of advanced practice nursing competencies in cancer care to identify obstacles to their full implementation. Background Internationally, the implementation of advanced practice nursing roles depends on the context and environment, which shape the definition, scope and competencies associated with these roles. Methods Nurses participated in two rounds of an online Delphi survey about the competencies of advanced practice oncology nurses. The threshold for expert consensus was set at 75%. Results Eleven competency domains were proposed; all yielded consensus of over 75%. However, for 57.8% of the specific competencies proposed in round 1 and for 62.2% in round 2, there was no consensus on which were applied in practice. There was more agreement on the competencies applied in the domains of direct clinical practice, consultation and collaboration and interprofessional relations than in dimensions such as health care promotion, quality improvement, evidence‐based practice and research. Barriers related to unimplemented competencies were identified. Conclusions The competencies applied in advanced practice nursing reflect incomplete development of these roles. Domains related to direct clinical practice, consultation and collaboration and interprofessional relations are relatively well developed, whereas those related to leadership, research, evidence‐based practice and quality improvement are not. The identified barriers hindering implementation of some competencies can inform strategies to develop this role in cancer care. Implications for Nursing Management Hospital administrators and nurse managers should reflect and be mindful of the development of advanced practice nurse (APN) competencies along with the challenges associated with implementing advanced practice roles.
