Complement Split Products C3a/C5a and Receptors: Are They Regulated by Circulating Angiotensin II Type 1 Receptor Autoantibody in Severe Preeclampsia?

Abstract: Background: This study measured the serum levels of complement component (C)3a and C5a and the placental expressions of C3a receptor (R) and C5aR to determine a potential correlation with circulating angiotensin II type 1 (AT1) receptor agonistic autoantibody (AT1-AA) in severe pre-eclampsia. Methods: A total of 118 women were recruited and divided into 2 groups: the control group (normotensive preterm pregnancies, n = 66) and severe pre-eclampsia group (n = 52). Levels of C3a, C5a and AT1-AA in serum were mea… Show more

“…C5a stimulated trophoblasts toward an anti-angiogenic phenotype by the increased expression of sFlt-1 and a decrease in PIGF (31). A lower C5aR protein expression was found in a larger study than Ma et al and only focused on severe preeclampsia compared to controls (29). This lower expression might indicate an independent factor in the pathogenesis of severe preeclampsia (29).…”

“…Lim et al found a decrease in C3aR mRNA expression in placental tissue of severe early-onset preeclamptic women compared to preterm control pregnancies, not affected by preeclampsia (28). Additionally, C3aR protein expression was significantly decreased in women with preeclampsia compared to controls (28,29). Decreased C3aR might indicate dysregulation of the complement system, inciting inflammation.…”

“…In addition, C5a interaction with C5aR on trophoblasts releases anti-angiogenic factors that impair normal placentation leading to preeclampsia and the associated placental insufficiency ( 139 ). Finally, C5a levels in women with preeclampsia positively correlate with the maternal autoantibody to the angiotensin Type 1 receptor, a potential contributor to the pathogenesis of preeclampsia ( 140 ).…”

“…In recent years, many studies have revealed that circulating C4d, C3a, C5a, and sC5b-9 levels were higher in patients with preeclampsia than in healthy pregnant women [3][4][5][6][7]21] . Meanwhile, complement-activated fragments, including C4d, C5a and MAC, were expressed at much higher levels in placentas of patients with preeclampsia than in normal pregnant women [8][9][10][11] .…”
