Complete morphofunctional oral rehabilitation by physiological increase of occlusal vertical dimension according to computerized mandibular scanner

Abstract: Increasing the occlusal vertical dimension (OVD) is often indicated in complex oral rehabilitation to gain restorative space and improve the occlusal relationship and aesthetics. The effect of increasing the OVD on lower facial height and facial aesthetics is not well understood and evaluated. The authors present the philosophy of the neuromuscular concept and illustrate it through a case report of a 58-year-old female patient who presented severe deep bite, bruxism, and temporomandibular disorders (TMDs). “…A study by Constantinescu et al [32] indicates that restoring the lost OVD can improve occlusal relationships and aesthetics without necessarily exacerbating TMDs. The study highlights a case where a 7 mm increase in OVD harmonized the facial profile and functional occlusion, with no adverse TMD symptoms reported.…”

“…Loss of OVD can also affect patient function, comfort, and esthetics. 23 Therefore, it is prudent to determine the magnitude of OVD loss. The latter is traditionally done with a set of calipers or a Willis gauge; alternative methods have been described that may provide a higher level of accuracy for this estimate.…”

“…A recent literature review indicates that there is strong clinical evidence that Rest Mandible Position (RMP), Rest Vertical Dimension (RVD), and the Interocclusal Dimension (IOD) are to be found within a three-dimensional spatial "range" largely determined by the active adaptation of the neuromuscular system [15]. This adaptation is extensive, but in specific cases, probably associated with other not necessarily occlusal components, dimensional variations may go beyond the limits of muscular compensation and induce occlusal instability and/or acute and chronic suffering [16,17].…”
