Complete motor recovery after acute paraparesis caused by spontaneous spinal epidural hematoma: case report

Abstract: BackgroundSpontaneous spinal epidural hematoma is a relatively rare but potentially disabling disease. Prompt timely surgical management may promote recovery even in severe cases.Case presentationWe report a 34-year-old man with a 2-hour history of sudden severe back pain, followed by weakness and numbness over the bilateral lower limbs, progressing to intense paraparesis and anesthesia. A spinal magnetic resonance imaging scan was performed and revealed an anterior epidural hematoma of the thoracic spine. He … Show more

“…MRI is a noninvasive tool that produces accurate findings. It can show both the position and the length of the hematoma along with the impact on the spinal cord [5].The SSEH is mostly located in the thoracic region as shown in this case report. This is due to the prominence of epidural venous plexus the thoracic spine [1].…”

“…18 On the contrary, some authors have mentioned conservative management followed up with imaging studies in cases of spontaneous resorption of hematoma leading to spontaneous recovery of neurological deficit. 22,23 The patient in our report denied history of trauma to his back. He was on antiplatelet therapy (aspirin) daily as he had past medical history of myocardial infarction and on a cardioselective β-blocker for blood pressure control.…”

“…[27][28] In follow up in only the incomplete paraplegic patients, it was possible to change paraplegia since complete sensory recovery was seen in no case of complete paraplegia. This observation supports the findings of the study of Yilmaz et al 29 Regarding the return of bowel and bladder function in all the cases this was autonomic in complete paraplegia.…”
