Complicated Total Knee Arthroplasty in a Patient with Klippel-Trenaunay-Weber Syndrome

Abstract: Case A 66-year-old Caucasian man with Klippel-Trenaunay-Weber syndrome (KTWS) presented with chronic changes related to the KTWS, along with worsening pain and motion associated with residual damage from an episode of spontaneous septic arthritis that occurred 1 year prior. He underwent total joint arthroplasty with a rotating hinged knee implant. Conclusion Arthroplasty is a treatment option for patients with KTWS; however, there are risks that must be considered. This case report outlines the management of… Show more

“…But, TKA is an effective option for severe knee arthritis caused by KTS 9 . These patients have a high risk of wound complications, bleeding, and infection 4,8,9 . Therefore, a thorough preoperative examination with the involvement of a multidisciplinary team comprising of vascular surgeons, infectious disease physicians, and dermatologists is considered to be necessary.…”

“…Because of the existence of vascular malformation, especially when the vascular malformation is in the joint itself and involves the synovium around the joint or the surgical inci-sion approach, the risk of bleeding can be extremely high. The abnormal proliferation of joint bones can lead to changes in anatomical structure, which makes the joint prosthesis unable to match well.Literature review shows that the number of reports for knee arthroplasty combined with KTS is limited [6][7][8][9][10] . Total knee arthropathy (TKA) is an option for orthopaedic treatment in KTS, but bone deformity makes it challenging; thus, we used 3dimensional (3D)-printed patient-specific instruments (PSI) to assist the surgery. The patient was informed that data concerning the case would be submitted for publication, and he provided consent.…”

“…However, these procedures were only indicated in refractory cases and the risks of the procedures were considered, such as wound complications, postoperative anemia, cardiovascular complications, infections, and thromboembolic events. [10][11][12][13][14][15]…”

“…Although indications for orthopaedic intervention in KTS are limited because limb length discrepancies are generally tolerated 2 , hypertrophic arthritis and recurrent hemarthroses can occur leading to joint degeneration. Preoperative evaluations, surgical protocols, and outcomes for total knee arthroplasty in patients with KTS have previously been described 3,4 . Available literature on total hip arthroplasty (THA) in patients with KTS is limited to 2 case reports 5,6 .…”
