Complication of improper management of sodium hypochlorite accident during root canal treatment

Abstract: Sodium Hypochlorite (NaOCl) is a common irrigation solution used in root canal treatment. It has strong antibacterial and tissue dissolving properties. Nevertheless, it has some serious complications, some of which are life-threatening. A young male presented with severe chemical burn of the right infraorbital area and partial necrosis of the hard palate resulting from extrusion of NaOCl during root canal treatment of the upper right 2nd molar tooth. The patient had a facial scar, and mucosal damage healed nea… Show more

“…Chlorhexidine and sodium hypochlorite proved to be active against common bacterial and fungal pathogens at lower concentrations compared to HybenX ® . Nonetheless, it has to be noted that Hybenx ® is actually administered undiluted, while chlorhexidine is usually applied in solutions ranging from 0.02% to 2% [7][8][9], and sodium hypochlorite 0.02-5.25% [10][11][12]. The activity of HybenX ® against S. aureus and E. faecalis appears to be of particular relevance due to the role of these pathogens in some periodontal infections (e.g., chronic and aggressive periodontitis) [13,14].…”

“…The crystal violet coloration used in the OD value test not only showed color changes occurring in the cell but also the secang solutions color attached to the surface of the plate; therefore, it may have overestimated the number of microorganism cells attached [7,14,15].…”

“…2.5% NaOCl can be recommended, as it has the same effectiveness as 5.25% NaOCl but is less toxic [6]. There has been a report of NaOCl extrusion into periapical tissues including acute pain, swelling, hemorrhage, numbness, and secondary infection [7].…”
