Complications after Single versus Dual Chamber Pacemaker Implantation

Abstract: To compare the complication rate in patients having a dual chamber versus a single chamber pacing system, 337 consecutive procedures performed during a 3-year period were analyzed prospectively. Two hundred fifty-eight patients (77%) received a VVI pacemaker and 75 (23%) a DDD unit. Thirteen VVI (5%) and 4 DDD (5.3%) needed reintervention. Lead displacement with reoperation was required for three ventricular leads (1%) and one atrial lead (1.3%). Infection occurred in two VVI units (0.77%) and one DDD (1.33%) … Show more

“…Though recent studies have shown complication rates were no higher for dual than for single chamber systems,4 12 our study suggests that late complications are more often observed with dual chamber systems, in keeping with a previous study 3. We speculate that subsequent studies may show that late complications are most commonly observed after elective unit replacement of dual chamber systems.…”

“…The infection rate in our study (0.55% after first implant) compares with 0.5% in Hill’s study,8 0.69% in a report by Matsuura et al ,9 and 0.9% in Mueller’s study 12. In a large study by Frame et al concerning 5089 patients over a 20 year period the infection rate was 1.06% 13.…”

“…Atrial lead displacement occurring within six weeks of upgrade was seen in eight patients (18%), compared with 11 of 286 patients who underwent dual chamber implants at this centre between 1987 and 1993 (3.8%),16 and with a standard atrial displacement rate of 1%17 to 5% 7. The reason for the high incidence of atrial displacement at upgrade is not immediately apparent.…”

“…Mueller et al 19 have recently challenged the highest indices of complication attributed to DDD,C pacemakers by showing results similar to those of the ventricular systems (VVI,C).…”
