Comprehensive Algorithm for Skull Base Dural Lesion and Cerebrospinal Fluid Fistula Diagnosis

Abstract: The four patterns of the new diagnostic algorithm described in the present study enable physicians to reliably clarify suspicions of dural lesions and CSF fistulas and aim to help them choose the best possible management. Each pattern uses the optimal combination of CSF tests and radiological imaging to reach a synergistic effect for precisely detecting dural lesions or CSF fistulas. Accordingly, this improves surgical decision-making when necessary.

“…CT scans in general, and high-resolution CT (HRCT) scans in particular, produce the best definition of bony structures and are considered to be the method of choice for the investigation of basal skull defects by many authors (118,128). An added advantage, in contrast to other techniques discussed below, is that HRCT is a noninvasive technique.…”

“…85 Another protein proposed as a potential marker for the diagnosis of CSF rhinorrhea is b-trace protein, or bTP. 4,45,[83][84][85]98,103,112 It is the second most abundant protein in human CSF following albumin 83 and is mainly produced in the epithelial cells of the choroid plexus and in the leptomeninges in the CNS. 83,103 The CSF concentration of bTP is approximately 35-fold higher than in plasma.…”
