Comprehensive rehabilitation of the child with osteogenesis imperfecta

Abstract: Children with osteogenesis imperfecta (OI) that results in considerable deformity are often viewed as poor candidates for aggressive physical therapy and rehabilitation. To determine if this view is realistic, we have entered almost 50 children with OI type III and OI type IV into a comprehensive graduated rehabilitation program, based at the National Institutes of Health, but designed to be implemented by continuing involvement of community resources. Children are begun in the program early with emphasis on g… Show more

“…This condition had been present since birth without a known etiology. The same observations have been reported by Binder et al 8 Manual muscle strength testing is hypothesized as a valuable tool for the clinical assessment of patients with neuromuscular problems. Barr et al 19 studied the inter-and intratester reliability in Duchenne and Becker muscular dystrophy and found high reliability (intertester correlation coefficient: 0.90; intratester correlation coefficient: 0.80).…”

“…Improvement of functional abilities is mainly focused on sitting, standing, and ambulation. 8,9 Our purpose was to investigate impairment patterns in OI, such as range of joint motion and muscle strength, as well as disability or functional limitation items, such as functional skills and the amount of parental assistance, in correlation to the different disease types. We also investigated correlations between impairment and disability items.…”

“…The literature surrounding therapy interventions, including physical therapy and occupational therapy, recognizes the importance each specialty has in enhancing function and independence for children with OI 25,26. The long-term goal is that children live independently and develop the life skills to enable them to direct their own care 21…”

“…Important issues involve the development and maintenance of muscle strength in the upper and lower extremities, the prevention of joint contractures at several joints including the shoulders, hands, hips, and feet, poor joint alignment, and disturbances of gait and low endurance in those who ambulate. To approach optimal rehabilitation and maintenance of function, children were classified as to the severity of disease and potential for rehabilitation and a specific program was outlined for each group [230]. Modalities included custom-molded seats to align hips, knees, and ankles to prevent stress in the spine, therapeutic water activities, soft tissue mobilization techniques to control joint contractures, and specific exercises to strengthen muscle groups, improve posture and balance, and increase endurance.…”
