Computer simulations of peritoneal fluid transport in CAPD

Abstract: To model the changes in intraperitoneal dialysate volume (IPV) occurring over dwell time under various conditions in continuous ambulatory peritoneal dialysis (CAPD), we have, using a personal computer (PC), numerically integrated the phenomenological equations that describe the net ultrafiltration (UF) flow existing across the peritoneal membrane in every moment of a dwell. Computer modelling was performed according to a three-pore model of membrane selectivity as based on current concepts in capillary physio… Show more

“…44,45 The loss of AQP1-mediated free-water transport has been suggested as a potential cause of UF failure in long-term PD patients. 46,47 Admittedly, this study does not allow ruling out a dysfunction or biochemical modification of AQP1.…”

“…have classically been considered to be <0.1 (10). The continuous endothelium lining the peritoneal capillaries can be functionally described in terms of a three-pore model (TPM) (16,17). In the TPM, approximately 90% of the peritoneal UF-coefficient (LpS), and 99.5% of the total pore area available for solute transport, is accounted for by small pores, and 5-8% by large pores, occupying <0.5% of the total pore area.…”

“…On the other hand, however, VEGFinduced neoangiogenesis cannot be viewed as the sole culprit of the peritoneal membrane dysfunction. Computer simulations suggest that an increase in peritoneal exchange surface area alone will not account for a massive decrease in drained volumes (Rippe et al, 1991). One has to bear in mind, however, there exists a tight link between angiogenesis and fibrosis (Wynn, 2007).…”
