Concomitant conus medullaris arteriovenous shunts and sacral dural arteriovenous fistulas: pathophysiological links related to the venous drainage of the lesions in a series of five cases

Abstract: We discuss the pathophysiological link between scAVS and sDAVF on the basis of the rarity of the DAVF, the uncommon association between scAVS and sDAVF, the presence of sDAVF in young patients, and the venous hypertension created by the venous drainage towards the sacral area responsible for angiogenesis creating the dural shunt.

“…According to our review about the coexistence of multiple types of spinal vascular lesions, the collected data in this review included demographic data (i.e., patient gender and age), symptoms and signs, type with location of each fistula, synchronous or metachronous occurrence of these fistulas, the presence of sharing the same venous drainage, treatment, and neurological outcome of the patients [ Table 2 ]. [ 2 , 4 - 7 , 9 , 10 , 12 , 13 , 15 , 16 , 19 , 20 ] Our search found 18 cases including our case with 36 spinal vascular lesions. There were 13 men (72.2%) and 5 women (27.8%).…”

“…In addition, Rosi et al . [ 19 ] reported five cases of concomitant conus AVM or AVF and sacral DAVF and assumed that the concurrent lesions are the pathophysiological linked. Most dural fistulas were found in the area in which the venous drainage of the conus lesions ended.…”

“…The co-existence of different types of spinal cord arteriovenous shunts is very rare. 60 Concomitant existence of spinal lipoma and acquired spinal AVF is also very rare. The lipoma releases angiogenic factors that are involved in the development of the spinal AVF.…”

“…Through beautiful case illustrations, Rosi and colleagues describe the rare association between conus medullaris arteriovenous shunts and lumbosacral dural arteriovenous fistulas (DAVFs) 10. In the event of a thoracolumbar AVM with caudal venous drainage, the authors recommend further investigation to rule out an associated lumbosacral DAVF.…”
