Concordance of regional hypoperfusion by pCASL MRI and 15O-water PET in frontotemporal dementia: Is pCASL an efficacious alternative?

Abstract: Highlights ASL is an alternative to 15 O-water for identifying hypoperfusion in FTD patients. ROI-based perfusion by ASL and 15 O-water were strongly correlated (R > 0.75). Hypoperfusion patterns identified by 15 O-water and ASL were in good agreement. Careful selection of the reference region is required to avoid erroneous results.

“…According to the aforementioned quality assessment tool [ 19 ], all included studies were rated fair to good. Nineteen (58%) comprised samples of subjects with SCD plus and patients with NCD [ 21 24 , 26 , 30 32 , 34 , 35 , 37 , 38 , 43 , 46 52 ], including mild cognitive impairment (MCI), AD, frontotemporal lobar degeneration (FTLD), DLB and vascular cognitive impairment (VCI), according to the corresponding diagnostic criteria [ 17 , 53 63 ]. Given that subjects with SCD plus have “features that increase the likelihood of preclinical AD” [ 17 ], we report them as a distinct subgroup belonging to the spectrum of cognitive decline.…”

“…Finally, the reliability of VSASL to detect between-region CBF variations could be facilitated with a direct comparison with 15 O-water PET as a gold standard modality for CBF mapping ( Fan et al, 2016 ). Hybrid PET/MRI would be ideally employed to validate CBF quantification by VSASL with simultaneous measurement by 15 O-water PET to ensure capturing the same physiologic state ( T. Ssali et al, 2022b ; Zhang et al, 2014 ).…”

“…Neurologically healthy participants were recruited through advertisements or the volunteer pool. PET and MRI data were collected at the Lawson Health Research Institute as part of a clinical study involving patients with frontotemporal dementia 27,28 . Complete data sets were acquired from 13 individuals (64 ± 10 years, 80 ± 20 kg, 8M/5F; average injected activity, 730 ± 60 MBq [range 620–790 MBq]; average injected activity per body weight, 9.4 ± 2.4 MBq/kg [range 5.6–14.1 MBq/kg]).…”
