Background-Benign prostatic hyperplasia (BPH), a nonmalignant enlargement of the prostate, can lead to obstructive and irritative lower urinary tract symptoms (LUTS). The pharmacologic use of plants and herbs (phytotherapy) for the treatment of LUTS associated with BPH is common. The extract of the berry of the American saw palmetto, or dwarf palm plant, Serenoa repens (also known by its botanical name of Sabal serrulatum), is one of several phytotherapeutic agents available for the treatment of BPH.Objectives-This systematic review aimed to assess the effects of Serenoa repens in the treatment of LUTS consistent with BPH.Search strategy-Trials were searched in computerized general and specialized databases (MEDLINE, EMBASE, and The Cochrane Library), by checking bibliographies, and by handsearching the relevant literature.Selection criteria-Trials were eligible if they (1) randomized men with symptomatic BPH to receive preparations of Serenoa repens (alone or in combination) for at least four weeks in comparison with placebo or other interventions, and (2) included clinical outcomes such as urologic symptom scales, symptoms, and urodynamic measurements. Eligibility was assessed by at least two independent observers. Data collection and analysis-Information on patients, interventions, and outcomes was extracted by at least two independent reviewers using a standard form. The main outcome measure for comparing the effectiveness of Serenoa repens with placebo or other interventions was the change in urologic symptom-scale scores. Secondary outcomes included changes in nocturia and urodynamic measures. The main outcome measure for side effects or adverse events was the number of men reporting side effects.Main results-In this update 9 new trials involving 2053 additional men (a 64.8% increase) have been included. For the main comparison -Serenoa repens versus placebo -3 trials were added with 419 subjects and 3 endpoints (IPSS, peak urine flow, prostate size). Overall, 5222 subjects from 30 randomized trials lasting from 4 to 60 weeks were assessed. Twenty-six trials were double blinded and treatment allocation concealment was adequate in eighteen studies.

Die benigne Prostatahyperplasie ist die hä ufigste Erkrankung des ä lteren Mannes, bei 80 % aller Mä nner ü ber 70 Jahre lä ßt sich eine BPH nachweisen; therapiebedü rfig sind davon ca. 40-60 % [1]. Aufgrund der zunehmenden Altersstruktur wird der Anteil der mä nnlichen Bevö lkerung mit BPH-bedingten Miktionsbeschwerden in den nä chsten Jahren noch weiter zunehmen. So hä ufig diese Erkrankung auch ist, so vielfä ltig sind auch ihre Behandlungsmö glichkeiten. Gerade in letzter Zeit drängen immer hä ufiger neue medikamentö se Therapieansä tze und instrumentelle Therapieverfahren auf den Markt, deren Einsatz zunä chst einmal kritisch beurteilt werden sollte.Wä hrend die transurethrale Prostataresektion und die offene Adenektomie in der deutschen Urologieals Fallpauschale und Sonderentgelt honoriert -als Standard gelten, haben verschiedene andere Verfahren wie z. B. die Thermotherapie, die Laserablation und die Elektrovaporisation in letzter Zeit Erfolge zu verzeichnen [2]. Doch der Großteil der Patienten wird nicht invasiv behandelt sondern medikamentö s versorgt. Nur 1 % aller BPH-Patienten werden in Deutschland operiert [3], ganz im Gegensatz zu Lä ndern, in denen keine konservative Therapie erfolgt und nur operative oder die sog. minimalinvasiven Therapieverfahren ü blich sind. Bei der Pharmakotherapie der BPH haben in Deutschland die Phytopharmaka den grö ßten Stellenwert. Als wichtigste Wirkstoffe in diesem Indikationsfeld mü ssen Phytosterole (b -Sitosterin), Extrakte aus Sä gepalmenfrü chten, Brennesselwurzeln und Kü rbissamen genannt werden. Lange stand diese Prä parategruppe im Kreuzfeuer der Kritik, doch mittlerweile muß anerkannt werden, daß die Wirksamkeit der Phytopharmaka auch in internationalen Studien bewiesen ist [4] und die BPH-Symptomatik gegenü ber Placebo signifikant gebessert wird. Seit Einfü hrung des Internationalen Prostatasymptomscores (IPSS), bei dem der Patient 7 Fragen zur Symptomatik und 1 bezü glich seiner Lebensqualitä t beantworten muß, sind Studien untereinander vergleichbar geworden. Anhand dieses standardisierten Wirksamkeitskriteriums lassen sich nun auch Phytopharmaka mit a -Rezeptorenblockern und 5a-Reduktasehemmern vergleichen. Der Vorteil von Pflanzenextrakten ist ihr breites pharmakologisches Wirkprofil, das der multifaktorellen Genese der BPH besser gerecht wird als die monistisch wirkenden a -Blocker oder 5a-Reduktasehemmer. Außer einer antikongestiven Wirkung haben Phytopharmaka im Gegensatz zu 5a-Reduktaseinhibitoren keinen Einfluß auf die Prostatagrö ße [4, 5]. Da nur eine sehr vage Korrelation zwischen dem Volumen der Pro-130 P H A R M A F O R U M Zum Thema In einer multizentrischen Anwendungsbeobachtung wurden bei 6967 Patienten Wirksamkeit und Verträglichkeit eines lipophilen Sägepalmenfrü chteextraktes (Sita) untersucht. Prü fparameter waren der Internationale Prostatasymptomscore, das Restharnvolumen und der maximale Harnfluß. Nach 6monatiger Therapie reduzierte sich der IPSS von 18 ± 6 auf 10 ± 6 Punkte, der maximale Flow stieg um 3 ml/s an und das Restharnvolumen...

The study carried out on rats given orally the n-hexane lipido/sterolic extract of Serenoa repens (LSESR), supplemented with [14C]-labelled oleic or lauric acids or beta-sitosterol, demonstrated that radioactivity uptake in prostatic tissues shows the highest level in the case of administration of LSESR supplemented with [14C]-labelled oleic acid. This was clearly demonstrated on a rat with an induced fibro-muscular hyperplasia of the prostate and by quantitative measurements of radioactivity. Ratios of radioactivity in tissues compared to plasma show an uptake of radioactivity greater in prostate as compared to other genital organs, i.e. the seminal vesicles or to other organs such as liver.
