“Congealed waterlilly” sign: A new sonographic sign of liver hydatid cyst

Abstract: The ultrasonographic (US) appearance of hydatid liver disease is well established, but solid cysts have long defied US characterization. We present 23 solid cysts and describe a new feature called the “Congealed waterlilly” sign. As the physical characteristics of the hydatid fluid change from watery to a viscid gel, it becomes echogenic. The intact folds of the germinal layer trapped within the viscid matrix give rise to the appearance of curvilinear structures which no longer move with changing patient postu… Show more

“…This change results in the appearance of curvilinear structures and intact folds of the germinal layer, which are trapped within the viscid matrix. They also noted that this sign was strongly suggestive of hydatid cysts [6]. Our case showed heterogeneous images on both mammography and ultrasonography, which were consistent with the type IV sonographic classification by Gharbi et al (ultrasound not presented).…”

“…They added that this sign strongly suggested the presence of hydatid cysts. 12 The USG-guided FNAC is a preoperative diagnostic tool that can provide a safe, quick, affordable and accurate diagnosis. 13,14 Protoscolices, hooklets and laminated membranes can be identified without complications.…”

“…They have thin walls resembling the pericyst, with internal small daughter cysts appear on US scan. 3,6,7 (Figure 1). Simple cysts do not demonstrate internal structures, although multiple echogenic foci due to hydatid sand may be seen within the cyst by repositioning the patient (snow-storm sign).…”
