It results from a congenital focal obliteration of a proximal segmental or sub-segmental bronchus with normal development of distal structures. Because of proksimal bronchial stenosis, the distal bronchi became filled with mucus and form a bronchocele. Three dimensional (3D) volume rendering (VR) imaging was performed with 64-slice multidetector computed tomography (MDCT) equipment in a patient with CBA and bronchocele. 3D VR images clearly revealed branching nonenhancing tubular structure and the adjacent lung with features of air trapping. A diagnosis of bronchocele was made radiologically. We report a case of bronchocele documented by 3D image reconstruction and VR images using 64-slice MDCT. In this article we report that, the radiologic findings of the broncocele can be shown more clearly with 3D VR technique than standart CT imaging.K Ke ey y W Wo or rd ds s: : Lung; bronchi; tomography, spiral computed Ö ÖZ ZE ET T Konjenital bronş atrezisi (KBA) nadir görülen bir akciğer anomalisidir. Proksimal segmental veya subsegmental bronkusun konjenital tıkanıklığı ve distal yapıların normal gelişmesi sonucu oluşur. Tıkanıklık distalinde bronş içerisinin mukus ile dolması sonucunda bronkosel gelişir. KBA ve bronkosel bulunan bir hastaya 64 dedektörlü çok kesitli bilgisayarlı tomografi (ÇKBT) cihazı ile üç boyutlu hacim görüntülemesi (3D VR) yapıldı. Bu üç boyutlu hacim görüntüleri çok net bir şek-ilde bronkosele ait tubuler yapıları, kontrast maddeye gerek kalmadan dalları ile birlikte çevresin-deki akciğerin hava hapsi alanlarını da gösterdi. Bronkosel tanısı radyolojik olarak konuldu. Bu makalede 64 dedektörlü ÇKBT cihazı ile üç boyutlu hacim görüntüleri elde edilen bir bronkosel olgusunu sunuyoruz. Introduction Congenital bronchial atresia (CBA) is a rare airway malformation. No management guidelines exist because of limited evidence: treatment, surgical or conservative, is based on consensus and opinion. Objective: To review the experience of a pediatric tertiary center and provide additional data about nonsurgical management of CBA and its outcomes after a structured follow‐up, and to help formulate appropriate evidence‐based guidelines. Methods A retrospective analysis of clinical, radiological, surgical, and pathological data of all pediatric patients with suspicion of CBA referred to the surgical department of the Bambino Gesù children's hospital of Rome between December 2013 and 2019, along with a review of the literature. Results Among the 18 children initially included in the study, 2 were lost to follow‐up after radiological diagnosis, 4 underwent surgery for radiological suspicion of other pulmonary malformations. The final population is composed of 12 conservatively managed patients. At the end of the follow‐up (median: 29 months, range 3–61), 1 patient (8%) was symptomatic. Conclusion Conservative management for CBA appears to be safe. Surgery should be reserved for patients with symptomatic or complicated cases.
