Congenital diaphragmatic hernia: the prognostic significance of the site of the stomach

Abstract: Congenital diaphragmatic hernia is associated with a significant mortality, despite intensive treatment. The degree of pulmonary vascular hypoplasia is the main factor affecting mortality. Various features have been considered to determine the prognosis in these infants. In this study a series of 50 consecutive cases of left-sided diaphragmatic hernia has been reviewed. The site of the stomach (abdominal or intrathoracic), demonstrated radiologically and confirmed at operation, has been related to the final ou… Show more

“…Our study confirmed the pejorative significance of some predictors previously described in the literature, such as the early diagnosis, the presence of the stomach in the thorax and the existence of associated malformations [4±9, 19,20,27,28]. Actually, all survivors were diagnosed after 25 weeks of gestation.…”

“…Goodfellow et al [15] examined retro spectively the plain radiographs of the chest and abdomen of 50 neonates with CDH and found that 20 of 34 (59%) with an intrathoracic position of the stomach died, whereas 15 of 16 infants (94%) where this was not the case survived. In our study the stomach was in the thorax in all the fetuses.…”

“…Several attempts have been made to identify some useful predictors of outcome; however, these markers fail to predict exactly whether a fetus with CDH will survive. In previous studies, some prenatal parameters have been related to the neonatal prognosis, including (1) polyhydramnios, 1 (2) the presence of an intrathoracic stomach 16,27,28 or liver, 19,29,30 (3) the lung-thorax transverse area ratio (LTR), 18,31 (4) the lung-to-head ratio, 32 and (5) fetal breathing movement. 20,21,33 In the past decade, fetal hemodynamic changes have been evaluated with Doppler fl owmetry, such as Doppler fl ow measurement of velocity in the pulmonary artery 22,34 and color fl ow mapping in the mesenteric artery, 23 umbilical vein, and venous duct.…”
