Congenital Hydranencephaly/Porencephaly Due to Vascular Disruption in Monozygotic Twins

Abstract: Monozygotic twinning has been associated with a variety of vascular disruptive defects including congenital hydranencephaly/porencephaly. Data involving 24 cases of congenital hydranencephaly/porencephaly associated with twinning are reported. In these cases, the finding of a preponderance of monozygotic twins and the common association of a deceased co-twin support the hypothesis of a vascular disruptive etiology. These defects are presumed to be secondary to embolic phenomena or thromboplastin release from t… Show more

“…5,17,[22][23][24] The increase in congenital anomalies in MZ twins is thought to result from the twinning process or interfetal hemodynamic complications. 36 Malformations, deformations, or disruptions can occur during the formation of an MZ twin, resulting in structural anomalies. 7 Considering that MZ twinning is inherently more likely to lead to malformations, it is plausible that there is a greater occurrence of congenital anomalies in MZ than in DZ twins or singleton pregnancies.…”

“… 17,21–24,31,34 Several studies used same-sex twins as a crude index for MZ twins 5,17,22–24 . The increase in congenital anomalies in MZ twins is thought to result from the twinning process or interfetal hemodynamic complications 36 . Malformations, deformations, or disruptions can occur during the formation of an MZ twin, resulting in structural anomalies 7 .…”

“…Enbom [14] reported that 46% of the survivors suffered major morbidity or death, although he recognized the shortcomings of his retrospective study, and the report by Szymonowicz et al [41] is even more devastating. Numerous other investigators have reported that significant neurological damage may occur in the surviving twin, and many additional case reports may be found referred to in these publications [2,9,13,20,21,23,38,44,45]. Thus, Hoyme et al [20] outlined structural defects observed in children whose monochorionic cotwin died in utero.…”
