“…An analysis of 43,143 patients treated at 171 centers revealed that the hospitals with high risk-adjusted 30-day readmission rate also had higher 1-year all cause readmission rate (59.1% vs. 54.7%, respectively; p = 0.01) [24]. The HFDMP group in our study had a lower 1-year readmission rate (29.67%) compared to HF patients treated with standard care as reported in the TSOC-HFrEF registry (38.5%) [2] and in HF registries in other countries (e.g., 30.1% in IN-HF outcome registry in India [25] and 36% in registry in Saudi Arabia [26]). In comparison, the EHFS-2 registry in Europe reveals a 1-year morality rate of 21.9% for patients hospitalized with acute HF [27], and the IN-HF registry in India reveals a 1-year morality rate of 24.4% [25].…”

“…A study conducted by Chen et al [ 13 ] revealed that heart failure management programs had a lower 1-year re-admission rate (29.67%) than patients who dealt with regular treatment (38.5%). In other countries of HF registries, such as the HF outcome registry in India, the 1-year re-admission rate was 30.1% [ 14 ], and in the Saudi Arabia registry, it was 36% [ 15 ]. On the other hand, in patients hospitalized with acute HF in Europe, the EHFS-2 registry shows a 1-year mortality rate of 21.9% [ 16 ].…”

“…In the second study, researchers approached 197 patients with CHF soon after admission to King Abdullah Medical City in Jeddah, Kingdom of Saudi Arabia, non-randomly assigning 121 to a HFP and 76 to a usual care group. 25 Outcomes examined were length of index stay, in-house mortality, and one-year re-admission rate. The HFP consisted of a 4-day clinical pathway during hospitalization that involved careful monitoring and completion of a daily checklist by a nurse coordinator (which included patient education), a 72-hour post-discharge telephone call, and early post-discharge appointments with a cardiac nurse specialist and cardiologist.…”
