Consensus Statement on Round Window Vibroplasty

Abstract: Round window (RW) vibroplasty has been established as a reliable procedure that produces good and stable results for patients with conductive or mixed hearing loss. The experience gained over the past few years of the authors' more than 200 implantations has led to consensus on several key points: (1) a wide and bloodless access to the middle ear with facial nerve monitoring, (2) the careful and correct identification and exposure of the round window membrane, (3) a good setup for efficient energy transition o… Show more

“…In contrast to conventional hearing aids, which often introduce signal quality loss and feedback, these devices are less prone to these problems. Moreover, in patients with diseased or previously operated middle ears, in which such problems are often exacerbated, these devices have been proved to be suitable [9,10,[12][13][14][15][16][17].…”

“…The coupler and FMT placement were chosen intraoperatively based on surgical findings and could not be randomized. Additionally, different surgical methods, such as reinforcing the eardrum with cartilage, placing cartilage over the implant, the use of Silastic sheeting and placing tissue between the RW membrane and the FMT [Beltrame et al, 2014] were used, based on the individual patient pathologies and anatomical preconditions. Coupling of middle ear implants is very important for proper sound transfer following middle ear reconstruction.…”

“…Six patients referred to the sound quality as being very good, two as good, only one rated the sound quality as moderate, and none of them scored the quality of sound as bad or very bad. Lower Figure 3 represents mean values of the HISQUI 29 .…”

“…As in our series, some patients are still users despite having the extruded wire in the EAC. It has been recommended to drill a deep groove for placing the conductor link in open cavities (29). This was carefully performed in all our patients.…”
