Contrast-induced acute kidney injury and mortality in ST elevation myocardial infarction treated with primary percutaneous coronary intervention

Abstract: CI-AKI is a frequent and serious complication of STEMI treated by pPCI. The RIFLE definition is the most accurate definition to identify patients with CI-AKI at high risk of mortality or haemodialysis.

“…Patients presenting with STEMI are prone to develop CI-AKI after primary PCI, and high thrombogenic state and inflammation are among the leading causes [5]. Hyperglycemic patients presenting with STEMI have higher coronary thrombus burden compared with thrombi from normoglycemic counterparts.…”

“…The highest serum creatinine level within 2 days after coronary angiography was used as post PCI creatinine to diagnose CI-AKI. The diagnostic criteria of CI-AKI was an elevation in serum creatinine ≥ 27 μmol/l or ≥ 50 per-cent over baseline within 48 h after contrast media injection [5]. Then the association between continuous metformin therapy in the perioperative period and incidences of CI-AKI was examined.…”

“…In patients with acute myocardial infarction with ST-elevation (STEMI) percutaneous revascularization with primary PCI (pPCI) represents the mainstay of acute treatment. The incidence of CI-AKI in patients treated with pPCI during STEMI varies from 10.5% to 18.3% depending on the definition used for CI-AKI, with a significant impact on mortality (Silvain et al, 2018).…”

“…Contrast volume media is an important risk factor for CIN. 4,5 Because of the longer duration of coronary interventions and repeated cine-angiographic views, we proposed that the use of contrast media volume in patients with large thrombus burden might be higher than the other patients. Demonstrating the contrast media volume in table 2 and performing logistic regression analysis after stratifying contrast media volume could better represent the association.…”
