Contribution of laparoscopy in the abdominal tuberculosis diagnosis: retrospective study of about 11 cases

Abstract: Abdominal tuberculosis is still frequent in Tunisia. Because of its non-specific clinical presentation and the limited means of investigation, a laparoscopy with biopsy should be practiced as first line diagnostic tool in case of doubtful abdominal tuberculosis. The earlier the diagnosis is established and an adapted antituberculosis treatment is started, the better the prognosis is.

“…Laparoscopic diagnosis with biopsy allows for fast and accurate diagnosis of TBP, which is important for early treatment and better patient prognosis (13). Although in certain situations, laparoscopic diagnosis alone may provide false positive rates in 18%, these are avoided when supported with histopathology and ascitic fluid analysis (14).…”

“…This patient's T-SPOT was negative on two occasions; the results may be due to condition related to active tuberculosis or may be because of the use of corticosteroids. Hence, the diagnosis for this type of special case required a peritoneal biopsy performed under direct visualization [7,[19][20][21][22]. The patient had refused to undergo invasive peritoneal biopsy procedure at the previous health center.…”

“…Diagnostic laparoscopy prompts the diagnosis and can reduce the delay which can increase the morbidity and can lead to unnecessary laparotomies while improving the outcome [30][31]. Despite all the benefits, some limitations like a deceiving view leading to mis-diagnosis regardless of the experience of surgeon are mentioned by few studies [32][33][34][35].…”
