Contribution of Serum Cytomegalovirus PCR to Diagnosis of Early CMV Primary Infection in Pregnant Women

Abstract: (1) Background: What is the role of serum CMV PCR in the diagnosis of recent primary infection (PI) in pregnant women when IgG avidity is uninformative? (2) Methods: Retrospective cohort study to compare serum versus whole blood CMV PCR. (a) Qualitative assessment: CMV PCR was performed on 123 serum samples and 74 whole blood samples collected from 132 pregnant women with recent CMV PI. PCR positivity rate was used to calculate sensitivity in serum and whole blood. (b) Quantitative assessment: CMV PCR was perf… Show more

“…Therefore, when early primary hCMV infection is suspected in a pregnant woman (before seroconversion and the appearance of testable IgG levels), polymerase chain reaction (PCR)-based testing using a serum blood sample, or preferentially whole blood, can be performed to confirm primary infection. Up to day 15 of primary infection, hCMV PCR has a positive predictive value of 89% and 100% for tests performed on serum and whole blood samples, respectively [ 45 ].…”

“…It is therefore likely that we underestimated PCR positivity rate. Our team recently highlighted performances and limits of CMV PCR in serum of immunocompetent patients [23]. By contrast to CMV positive-IgM, when CMV PCR is positive, whatever the viral load, NPI is confirmed.…”
