Controversies in Serotonin Syndrome Diagnosis and Management: A Review

Abstract: Over the past few years, Serotonin Syndrome (SS) has become a significant clinical concern. Over the last decade, United States saw a surge in antidepressant use. SS characteristically presents as the triad of altered mental status, autonomic dysfunction and neuromuscular excitation. Symptoms vary from patient to patient with mild cases presenting with subacute symptoms and severe cases progressing rapidly to death. Due to the protean manifestations of the syndrome along with non-specific prodromal, SS can eas… Show more

“…Recent reviews of SS have provided a comprehensive discussion of its clinical manifestations and diagnosis . Although a potentially life‐threatening condition, severe cases of SS are generally easily recognized and involve a constellation of symptoms that include some combination of autonomic dysfunction, mental status changes, and/or neuromuscular hypertonicity.…”

“…Symptoms such as anxiety and akathisia can be misattributed to the patient's mental state, while hypertension, diarrhoea, or tremor can easily be dismissed as part of a systemic or metabolic disorder unrelated to drug therapy. 101 There are 3 diagnostic classification systems available, the Sternbach (SC), Radomski (RC), and Hunter Serotonin Toxicity Criteria (HSTC), all of which try to reflect symptoms and symptom constellations that are felt to be characteristic of serotonin toxicity and therefore SS. The most recent and commonly used criteria are the HSTC, which are set of decision rules that focus on the classic features of generalised clonus (inducible, spontaneous, and ocular), agitation, diaphoresis, tremor, and hyperreflexia 41 (Figure 3).…”
