Convalescent Pulmonary Dysfunction Following Hantavirus Pulmonary Syndrome in Panama and the United States

Abstract: The objective of this study was to document persistent pulmonary symptoms and pulmonary function abnormalities in adults surviving hantavirus pulmonary syndrome (HPS). Acute infection by most hantaviruses result in mortality rates of 25–35%, while in Panama the mortality rate of 10% is contrasted by an unusually high incidence. In all types of HPS, the viral prodrome, cardiopulmonary phase due to massive pulmonary capillary leak syndrome, and spontaneous diuresis are followed by a convalescent phase with exert… Show more

“…In contrast, circulating serum and plasma VEGF levels only increased 11–20 days after-admission (Figures 3(a), and 3(b)). Increased circulating VEGF at late times after infection is consistent with vascular remodeling and repair that occurs during recovery phases of other causes of acute pulmonary edema and may coincide with HPS convalescence [32, 33, 36, 39, 44, 46, 48, 49, 52, 64]. These findings are similar to studies of patients with high-altitude-induced pulmonary edema where pulmonary VEGF levels are associated with acute disease and plasma levels of VEGF only become elevated during recovery [33].…”

“…However, there are reports of possible longterm consequences of hantavirus infections, indicating that treatment of acutely infected patients might also be beneficial for patients who recover from HFRS/HCPS. There are several reports on possible long-term consequences of HFRS [2], and recently this has also been investigated for HCPS [62,63]. The long-term consequences reported are most often mildly affected renal and lung function and blood pressure [62][63][64][65][66], but chronic hormonal deficiencies requiring hormone-replacement therapy have also been observed [2,67].…”
