Conventional Trabeculotomy versus Gonioscopy-Assisted Transluminal Trabeculotomy: A Retrospective Cohort Study

Abstract: Background: Conventional trabeculotomy (CT) is performed in an ab-externo manner with at most 120 degrees of incision area of Schlemm’s canal (SC). Recently, gonioscopy-assisted transluminal trabeculotomy (GATT), which makes possible a 360-degree incision area of SC in an ab-interno manner, is introduced. The purpose of this study was to compare surgical outcomes for CT and GATT with and without simultaneous phacoemulsification over 24 months and to identify factors associated with surgical success. Results: P… Show more

“…Corneal oedema occurred in 3 eyes (8%) in the phaco‐GATT group and resolved with conservative treatment. Transient IOP elevation (defined as IOP ≥25 mmHg that occurred between 1 week and 1 month postoperatively, followed by reduction to ≤21 mmHg) (Takata et al, 2021) occurred in 2 of 36 eyes (5.6%) in the phaco‐GATT group .…”

“…Table 2 compares the results of these studies with the phaco‐GATT group of our current study. Circumferential SC incisional procedures may further lower the resistance to outflow through the eye's natural outflow channels and have been shown to yield more IOP reduction than smaller extent incisions (Elhusseiny et al, 2019; Qiao et al, 2021; Takata et al, 2021). Our results report a higher IOP reduction, and a higher success rate compared to other studies that combined phaco with <180‐degree angle incision, with the exception of the study by Kiuchi et al, 2010 who performed laser iridoplasty in addition to the phacotrabeculotomy.…”

“…GATT is an MIGS surgery, firstly proposed by Grover et al [12] in 2014. This surgery is performed by 5-0 prolene suture or the iTrack™ 250-μm microcatheter to make a 360° incision of the trabecular tissue and the inner wall of Schlemm's canal for primary and secondary OAG [13][14] . It has been reported that the success rate of GATT alone can reach 60%-70%, while it can reach more than 90% combined with medication or cataract surgery [15] .…”
