Convergence Insufficiency in the Geriatric Population

Abstract: SIGNIFICANCE:The present study is the first population-based study to examine the prevalence of convergence insufficiency and its associations specifically in the geriatric population. Knowledge of the population-based determination of prevalence of this disorder in the elderly is necessary to support proper clinical diagnosis and management.PURPOSE: This study aimed to determine the prevalence of convergence insufficiency and its associated factors in a geriatric population.METHODS: In this study, all residen… Show more

“…In this prospective study using pre-specified diagnostic criteria for binocular vision anomalies, at baseline NSBVAs were common (33%), with the majority having convergence insufficiency (28%). A similar high frequency (21.5%) of convergence insufficiency was reported by Hashemi et al 30 in a geriatric population using the same diagnostic criteria (3-sign criterion). There was no significant change in the frequency of binocular vision anomalies after cataract surgery.…”

“…Rather, binocular vision conditions are considered syndromes and the diagnosis is based on a series of test results. 24,30 Thirdly, most previous studies were retrospectively designed, which precluded us from determining whether the post-operative binocular vision disorders are new or represent decompensation of previously existing conditions. In addition, there is a lack of use of a validated symptom survey in previous studies, and typically, only the symptom of diplopia was studied.…”

“…In addition, our study cohort does not represent the spectrum of adults with symptomatic convergence insufficiency, and therefore, our results are generalizable only to those with similar clinical and demographic characteristics. For example, approximately two-thirds of the adult participants in the vision therapy/exercise group were younger than 35 years, with few in the age range with the highest prevalence of convergence insufficiency (older than 60 years) 4 . It is unknown whether the associations we found would differ (stronger or weaker) in older persons.…”

“…Therefore, it would be helpful to find simple and preferably objective routine clinical methods to help identify MCI. Near point of convergence (NPC) refers to the maximum convergence capability of the eyes and consists of four distinct elements including tonic convergence, proximal convergence, accommodative convergence, and fusional convergence [17] . A receded NPC is the most important clinical sign used by optometrists to diagnose convergence insufficiency; the most prevalent binocular vision anomaly, especially in the elderly population [17] .…”
