Abstract: Background Oral anticoagulants (OACs) are commonly prescribed, have well documented benefits for important clinical outcomes but have serious harms as well. Rates of OAC-related adverse events including thromboembolic and hemorrhagic events are especially high shortly after hospital discharge.
