Cord Blood Units with Low CD34+ Cell Viability Have a Low Probability of Engraftment after Double Unit Transplantation

Abstract: Double unit cord blood (CB) transplantation (CBT) appears to augment engraftment despite only one unit engrafting in most patients. We hypothesized that superior unit quality, as measured by a higher percentage of viable cells post-thaw, would determine the engrafting unit. Therefore, we prospectively analyzed 46 double unit transplants post-thaw using flow cytometry with modified gating that included all dead cells. Using a 75% threshold (mean viability minus 2SD), 20% of units had low CD34+ cell viability, w… Show more

“…19 More recently, we have found that the percentage of viable CD34 ϩ cells after thaw can vary significantly according to the bank of origin, and poor viability units were unlikely to engraft in our analysis. 18 Querol et al have similarly reported variable quality between units. 20 This raises the possibility that part of the benefit of double-unit CBT is that, by transplanting 2 units, we increase the chance that at least 1 unit of good quality, and thus engraftment potential, is infused.…”

“…The quality of units can vary not only from unit to unit, but from bank to bank. 18 McCullough et al have reported unit quality problems in 56% of 268 units evaluated, and in 10% of cases these were considered to place the patient at risk. 19 More recently, we have found that the percentage of viable CD34 ϩ cells after thaw can vary significantly according to the bank of origin, and poor viability units were unlikely to engraft in our analysis.…”

“…13,14 There are also data using flow cytometry as a measure of CD34+ cell viability, with higher viability being predictive of engraftment in double cord transplantation. 15 We present the outcome of 104 patients enrolled on a single institution prospective trial of UCBT for relapsed/refractory hematologic malignancies. The primary purpose was to investigate the effect of TNC and CD34+ cell dose on engraftment and outcome.…”
