Objectives:To describe the clinical features and microstructural characteristics assessed by in vivo confocal microscopy (IVCM) in patients with adenoviral epidemic keratoconjunctivitis (EKC).Materials and Methods:The study included 20 eyes of 12 patients who presented to the Kocaeli University Medical Faculty, Department of Ophthalmology with complaints of watering, crusting, and stinging, were clinically diagnosed EKC, and were examined by slit-lamp biomicroscopy and IVCM during the prodromal phase and the punctate keratitis, deep epithelial keratitis, and subepithelial infiltration stages of EKC.Results:While biomicroscopic examination findings were normal during the prodromal period of EKC, IVCM showed an increase in Langerhans cell numbers in the subbasal plexus. After onset of clinical EKC, the punctate epithelial keratitis stage was characterized by findings of hyperreflective cell clusters in the basal epithelium layer, increased accumulation of Langerhans cells in Bowman’s layer, and hyperreflectivity in the anterior stromal layers. In the deep epithelial keratitis stage, the basal epithelial cells displayed peripheral hyperreflectivity and the hyperreflectivity of the anterior stromal surface increased and became more rounded. In the subepithelial keratitis stage, these findings persisted in addition to increased anterior stromal surface hyperreflectivity and focal round plaques.Conclusion:This study shows that the inflammatory process in the cornea starts in the prodromal period of EKC. Massive inflammation of the epithelium and stroma was observed in the active phase and focal changes were observed on the anterior stromal surface starting in the subepithelial infiltration period.

Ö ÖZ ZE ET T A Am ma aç ç: : Adenoviral konjonktivit görülen hastaların demografik ve klinik özelliklerinin değerlendirilmesidir. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Klinik olarak adenoviral konjonktivit tanımlanan hastalar değerlendirildi. Hastaların cinsiyeti, yaşı, etkilenen gözü, yakınlarında aynı göz belirtileri olan kişi-lerin olup olmadığı, havuz veya denize girme öyküleri kaydedildi. Biyomikroskopi ile ön segment muayenelerinde foliküler konjonktivit görülen hastalar konjonktiva hiperemisi, siliyer enjeksiyonu, kemozis, kapak ödemi, subkonjonktiva hemorajisi, kornea tutulumu ve psödomembran oluşumu açısından incelendi. Çalışmamızda adenoviral konjonktivitlerin daha çok hangi yaşlarda görüldüğü, bulaşıcılık durumunun olup olmadığı araştırıldı. Hastalarda kornea tutulumunun diğer faktörlerle ilişkisi değerlendirildi. B Bu ul lg gu ul la ar r: : Çalışma, 28 (%38,9) kadın, 44 (%61,1) erkek olmak üzere toplam 72 hasta ile yapıldı. Hastaların yaş ortalaması 19,35±15,27 (1-58) yıl idi. Muayene esnasında 16 (%22,2) hastanın sağ gözü, 15 (%20,9) hastanın sol gözü ve 41 (%56,9) hastanın her iki gözü etkilenmişti. Konjonktivitin ortaya çıkma şekli sorgulandığında 22 (%30,6) hastada bulaş yolu saptanmadı. Elli (%69,4) hastada yakın çevre kaynaklı bulaşma olduğu görüldü. Bulaşma görülen 50 hastadan 21 (%42) hastaya aile içinden, 3 (%6) hastaya yaşadıkları apartmanın sakinlerinden, 17 (%34) hastaya arkadaşlarından, 3 (%6) hastaya denizden, 4 (%8) hastaya daha önce gittikleri hastaneden, 2 (%4) hastaya kreşten bulaşıcılık olduğu öğrenildi. Korneal tutulum görülen 16 hastanın 11 (%68,8)'inde her iki gözde subepitelyal keratit izlendi. S So on nu uç ç: : Adenoviral konjonktivit, kişinin günlük faaliyetlerini etkileyerek yaşam kalitesini düşürebilir. Yüksek bulaşıcılık oranı olan bu konjonktivit tipinde hem klinisyenin hem de hastanın bulaşıcılık konusunda dikkatli olması öneril-melidir.

Adenovirüsler viral konjonktivitlerin en sık nedenlerini oluşturmakta ve kornea tutulumuna bağlı olarak ciddi morbiditeye neden olabilmektedir. Bu olgu sunumunda adenoviral keratokonjonktiviti olan üç hastanın klinik ve in vivo konfokal mikroskopik (İVKM) özellikleri tanımlamayı amaçladık. Tüm hastalarda biyomikroskopik muayenede subepiteliyal infiltratlar mevcuttu. İVKM ile bazal epitel hücreleri ve ön stromada hiperreflektif inflamatuvar hücreler ve subepitelial alanda dendritik hücre infiltrasyonu saptandı. Subbazal sinirler, arka stroma ve endotel tabakaları hücre morfolojisi, reflektivite ve hücre yoğunlukları açısından normal morfolojiye sahipti. (Turk J Ophthalmol 2015; 45: 119-121) Anah tar Ke li me ler: Epidemik keratokonjonktivit, adenovirus, subepiteliyal infiltrat, in vivo konfokal mikroskopi Adenoviruses are the most common cause of viral conjunctivitis and may lead to severe morbidity due to corneal involvement. In this report, we aimed to describe the clinical and in vivo confocal microscopic (IVCM) findings of three patients with adenoviral keratoconjunctivitis. All patients had corneal subepithelial infiltrates as observed on slit lamp biomicroscopy. IVCM revealed hyperreflective inflammatory cells in the basal epithelium and the anterior stroma together with subepithelial infiltrations of dendritic cells. Subbasal nerves, posterior stroma, and endothelium showed normal morphology, reflectivity, and cellular density.

Corneal Sensitivity may Decrease in Adenoviral Epidemic Keratoconjunctivitis—A Confocal Microscopic Study

Abstract: There may be a transient decrease in the corneal sensitivity during the course of epidemic keratoconjunctivitis. Confocal microscopy can help to evaluate the changes in the cornea during this period. Future studies are needed to understand the nature of this clinical finding.
