Rheumatoid arthritis is a prevalent worldwide disease, associated with an increased risk of multiple metabolic abnormalities that generate a higher disease burden. Objective: To gather the available evidence on the epidemiology, pathophysiology, current perspectives, clinical implications and prognosis of metabolic abnormalities in patients with rheumatoid arthritis. Methods: This is a narrative literature review. Search was conducted in PubMed, OVID, and Taylor & Francis databases, using the following MeSH terms: "Arthritis Rheumatoid", "Metabolic Diseases", and "Metabolic Syndrome". Results: This study describes the main metabolic manifestations of rheumatoid arthritis. Research has recognized that rheumatoid arthritis and metabolic abnormalities share pathophysiological mechanisms with an additive effect that increases cardiovascular risk. In that context, appropriate antirheumatic treatment can also impact on cardiovascular risk. Conclusion: There are metabolic abnormalities in rheumatoid arthritis patients that increase cardiovascular risk. Therefore, it is crucial to evaluate cardiovascular risk to provide appropriate comprehensive management to reduce morbidity and mortality in patients with this disease.

Atherosclerosis is the leading cause of cardiovascular disease among adults. The incidence of atherosclerosis increases significantly with age, which indicates the possible influence of aging mechanisms on the development of the disease, including changes in epigenetic factors caused by pathological activation of transposable elements. Triggers of atherosclerosis are also viral infections, which promote the expression of retroelements that stimulate the interferon response with the development of chronic inflammation. Activated retroelements also alter the regulation of immune system genes and epigenetic factors, including the pathological production of microRNAs and long non-coding RNAs. A promising direction for atherosclerosis treatment is the epigenetic impact on the expression of specific genes involved in the pathogenesis of atherosclerosis using small interfering RNAs. In this regard, the drugs inclisiran and olpasiran have undergone clinical trials and have shown their effectiveness. Therefore, it is important to search for new molecular targets in this direction, which can serve as transposons, which are sources of non-coding RNAs. Changes in the activity of retroelements during aging have a global regulatory effect on the functioning of the entire genome, contributing to the development of age-associated pathology. An analysis of the scientific literature made it possible to identify 29 microRNAs derived from retroelements, changes in the expression of which have been identified both during aging and atherosclerosis. These microRNAs can be used as tools for prolonging life and treating cardiovascular pathology. The results obtained also indicate that retroelements pathologically activated during aging cause the development of atherosclerosis.

Association between Mucosal Healing and Lipid Profiles in Patients with Ulcerative Colitis: A Cross-Sectional Study

Introduction: Recent meta-analysis showed that ulcerative colitis (UC) is a risk factor for cardiovascular disease (CVD). Dyslipidemia is a well-established risk factor for CVD. However, evidence regarding inflammatory bowel disease (IBD), including UC and lipid profiles, is limited. Additionally, no study has assessed the association between endoscopic activity and lipid profiles in patients with IBD. Therefore, we aimed to clarify the association between mucosal healing (MH) and lipid profiles in patients with UC. Methods: A total of 221 Japanese patients with UC were enrolled in this analysis. Total cholesterol (T-chol), triglyceride (TG), and high-density lipoprotein cholesterol (HDL-C) were divided into tertiles (high, moderate, and low) based on the distributions, respectively. Self-administered questionnaire was used to obtain information regarding medication for dyslipidemia. Complete MH and MH are based on Mayo endoscopic subscore 0 and 0–1, respectively. Results: The percentage of complete MH was 30.8%. In patients without medication for dyslipidemia, high HDL-C (>66 mg/dL) was significantly positively associated with complete MH (adjusted odds ratio [OR] 2.58, 95% CI: 1.04–6.64, p for trend = 0.037). In patients with nonproctitis, a positive association between HDL-C and complete MH was found (adjusted OR 3.54, 95% CI: 1.22–11.01, p for trend = 0.020). T-chol and TG were not associated with MH, regardless of medication for dyslipidemia. Conclusions: Complete MH was significantly positively associated with HDL-C in UC patients without medication for lipid. The disease extent might affect the association between complete MH and HDL-C.
