Coronary Disease Risk Curve of Serum Creatinine is Linear in Turkish Men, U-Shaped in Women

Abstract: Increasing serum creatinine values are associated strongly and independently with CHD risk in men but not in women in whom the risk curve is U-shaped. The phenomenon of low creatinine levels underlies some hitherto unexplained relevant observations, and low measurements may be attributed to inassayability secondary to involvement in autoimmune activation.

“…An association between serum ADMA and endothelial function was lacking also in patients with rheumatoid arthritis (34). From the distribution of apoB-containing lipoproteins and the positive linear association of serum creatinine with ADMA, it may be considered that both creatinine (35,36) and lipoprotein(a) (37) may have been further involved in the autoimmune complex formation in this sample, as previously shown by us to confer incident CHD risk.…”

“…This is consistent with a setting in which high adiponectin levels in a subset of the female sample were converted pro-oxidative to mediate endothelial dysfunction, acquiring attenuated atheroprotective effects, concomitantly with a reduced glomerular filtration rate. In view of our recent reports of higher CHD risk in women in the bottom creatinine quartile compared with the two intermediate quartiles [23,24] , immune complex formation with adiponectin may be suggested.…”

“…This may again manifest itself in one sex in the general population. Increasing serum creatinine values were strongly associated with CHD risk in men but not in women in whom the risk curve was U-shaped [19]. In the prediction of incident type-2 diabetes risk by baseline estimated glomerular filtration rate in the Insulin Resistance Atherosclerosis Study, other than the bottom quintile, the top estimated glomerular filtration rate quintile disclosed excess multi-adjusted risk compared with the referent 4th quintile [21].…”

“…Low serum creatinine levels (<0.7/<0.8 mg/dl) may represent a clue to the existence of autoimmune activation [19,20]. This may again manifest itself in one sex in the general population.…”
