Coronary Stent Fracture: A Recently Appreciated Phenomenon with Clinical Relevance

Abstract: In the stent era, in addition to restenosis, there are many important consequences deserving more attention. Firstly described in peripheral vascular interventions, it took several years for stent fracture to be known as an appreciable complication of coronary intervention. Especially with the introduction of drug eluting stents and the use of coronary stents in more complex cases, its prevalence has raised and new data have been published concerning its mechanism, predictors, diagnosis, clinical course and tr… Show more

“…A coronary stent fracture is a discontinuation of any part of the stent structure seen on angiogram [ 3 ]. Depending on the study and type of stent, the incidences of SF lie between 0.5% and 19% [ 4 7 ]. In a pathological study, the incidence rose as high as 29% [ 8 ].…”

“…These factors are all related to abnormal forces placed on the stent, creating metal fatigue and eventual fracture. Vessel tortuosity (especially in the right coronary artery), vessel calcification, ostial stent location, hinge motion, overlapping stents, increased stent length, smaller stent diameter, coronary aneurysm, and balloon overdilation are some of the known risk factors for SF [ 3 , 4 , 6 , 7 , 11 ]. The design of the Synergy due to its thin-strut and platinum chromium platform is said to incur flexibility and strength to prevent SF [ 3 , 9 ].…”

“…In addition, duodenal peristalsis and tumor compression can promote stent fracture occurrence. Operation-related factors, such as selecting stent size, passing the stent across the ampulla ( P = 0.09), and pre- or postdilation ( P = 0.10), may also contribute to stent fracture development [ 17 ]. Concerning stent patency time, no significant difference between the fracture and nonfracture groups was obtained ( P = 0.52).…”

“…Vascular stenting is widely used clinically with excellent clinical outcomes for the patient. Among various forms of cardiovascular stent surgeries, the overlapping stent technique has been frequently used to avoid incomplete stenting and secondary implantation [1][2][3]. In a study by Yamada and Koizumi [4], 102 consecutive sirolimus-eluting stents (SES) were implanted in 83 lesions, with 43 SES as multiple overlapping stents.…”
