OBJECTIVE: To describe the problem of obesity in Mexican American children and provide a method for identifying Mexican American children at risk for obesity. DESIGN: Cross-sectional and retrospective cohort study. SUBJECTS: Mexican American children, ages 5 ± 11 (kindergarten (K) ± 5th grade) were studied in cross-sectional (girls 463, boys 448) and retrospective (girls 124, boys 92) samples. MEASUREMENTS: Weights (kg) and heights (cm) were extracted from each child's school medical records. Measurements were made by the school nurses (one nurse per school). Body mass index (BMI [wah 2 ]) was calculated from obtained weights and heights. RESULTS: The prevalence of obesity in the cross-sectional sample was 27.4% for girls and 23.0% for boys. The highest prevalence was noted in 4th grade for both the girls (32.4%) and boys (43.4%). The 5 year incidence rates derived from the retrospective sample were 17.7% for girls and 21.1% for boys. The largest increase in the number of girls becoming obese was observed between K and 1st grade (15.6%). The largest increase for boys was found between the 1st and 2nd grades (13.2%). Logistic regression parameters were calculated to estimate the probability of becoming obese in 5th grade given a child's K BMI. Using the generated equation, BMIs in K equal to 16.5, 20.9, and 23.7 would have a 21.0%, 70.0%, and 91.0% probability of being obese by 5th grade. CONCLUSION: This study provides schools with a simple procedure for identifying Mexican American children at risk for obesity. This procedure may assist in the development and implementation of interventions aimed at averting the onset of obesity in this population.

Relationships between media use, body fatness and physical activity in children and youth: a meta-analysis

OBJECTIVE:To review the empirical evidence of associations between television (TV) viewing, video/computer game use and (a) body fatness, and (b) physical activity. DESIGN: Meta-analysis. METHOD: Published English-language studies were located from computerized literature searches, bibliographies of primary studies and narrative reviews, and manual searches of personal archives. Included studies presented at least one empirical association between TV viewing, video/computer game use and body fatness or physical activity among samples of children and youth aged 3-18 y. MAIN OUTCOME MEASURE: The mean sample-weighted corrected effect size (Pearson r). RESULTS: Based on data from 52 independent samples, the mean sample-weighted effect size between TV viewing and body fatness was 0.066 (95% CI ¼ 0.056-0.078; total N ¼ 44 707). The sample-weighted fully corrected effect size was 0.084. Based on data from six independent samples, the mean sample-weighted effect size between video/computer game use and body fatness was 0.070 (95% CI ¼ À0.048 to 0.188; total N ¼ 1722). The sample-weighted fully corrected effect size was 0.128. Based on data from 39 independent samples, the mean sample-weighted effect size between TV viewing and physical activity was À0.096 (95% CI ¼ À0.080 to À0.112; total N ¼ 141 505). The sample-weighted fully corrected effect size was À0.129. Based on data from 10 independent samples, the mean sample-weighted effect size between video/computer game use and physical activity was À0.104 (95% CI ¼ À0.080 to À0.128; total N ¼ 119 942). The sample-weighted fully corrected effect size was À0.141. CONCLUSION: A statistically significant relationship exists between TV viewing and body fatness among children and youth although it is likely to be too small to be of substantial clinical relevance. The relationship between TV viewing and physical activity is small but negative. The strength of these relationships remains virtually unchanged even after correcting for common sources of bias known to impact study outcomes. While the total amount of time per day engaged in sedentary behavior is inevitably prohibitive of physical activity, media-based inactivity may be unfairly implicated in recent epidemiologic trends of overweight and obesity among children and youth. Relationships between sedentary behavior and health are unlikely to be explained using single markers of inactivity, such as TV viewing or video/computer game use.

O presente estudo tem como objectivos identificar os factores que influenciam a prática da actividade físican os adolescentes portugueses. A identificação destes factores é um passo chave na identificação e afastamento de barreiras à prática da actividade física. Este estudo usa dados da amostra nacional do HBSC (Matos, Simões, Carvalhosa, Reis & Canha, 2000). A amostra nacional consistiu em 6903 alunos, de 191 escolas portuguesas seleccionadas aleatoriamente após estratificação por região, cobrindo todo o Portugal continental. Os alunos ficaram assim distribuídos pelas áreas educativas: 39.7% Norte, 24.7% Centro,25% Lisboa, 6.2% Alentejo e 4.4% Algarve. Dos 6903 alunos, 53% eram raparigas e 47% rapazes, 34.9% do 6.º ano, 37.5% do 8.º ano e 27.6% do 10.º ano.Os resultados, utilizando um modelo linear de regressão múltipla, revelam como variável explicativa mais forte o sexo. Tanto a idade como o sexo foram associadas à prática da actividade física com os rapazes e os mais novos a praticar mais. A percepção da saúde apareceu também associada à prática da actividade física, uma melhor percepção associando-se a uma maior prática. Foi também encontrada uma associação entre o tempo passado com os amigos fora das escola e as noites passadas com os amigos e a prática da actividade física. A associação encontrada entre a posse de automóvel de família e a prática da actividade física não foi inesperada, outros estudos referiram já a importância da mobilidade e acessibilidade na prática da actividade física em adolescentes.Se aceitarmos os já reconhecidos efeitos benéficos da actividade física na saúde e bem estar físico, social e psicológico dos adolescentes, estes resultados apontam claramente para a urgência de medidas a nível nomeadamente da família e da escola no sentido de apoiar os jovens na sua escolha de um estilo de vida activo e saudável tendo em consideração as diferenças de género.
