Correlation between Hepatitis B Virus Infection and Colorectal Neoplasia

Abstract: Background: Data about the association between hepatitis virus infection and colorectal neoplasia (CRN) are extremely limited. We examined the association between hepatitis B virus (HBV) and hepatitis C virus (HCV) infection with the risk of CRN. Methods: A cross-sectional study was performed on asymptomatic examinees who underwent a colonoscopy and serologic testing for hepatitis B surface antigen (HBsAg) and hepatitis C antibody (HCV Ab) between 2004 and 2015. Results: Of 155,674 participants who underwent s… Show more

“…Interestingly, the early‐onset cases were more likely to have a history of HBV infection in our cohort, which is consistent with the previous age‐specific analysis that the HBV‐related colorectal cancer risk decreases with aging in the HBV endemic regions. 29 , 30 The mechanism underlying the link between HBV infection and colorectal carcinogenesis has not been well understood at this moment. Previous studies have indicated that HBV may exist in extrahepatic tissues, such as the kidneys, lymph nodes, pancreas, and gastrointestinal tract.…”

“…Colorectal cancer has been shown to be closely associated with viral hepatitis B, which significantly increases the risk for colorectal carcinogenesis, especially in the Chinese population [ 53 ]; therefore, these patients are in need of frequent screening. A higher risk of CRC development was also shown in studies performed in the Korean population [ 54 , 55 ].…”

“…In particular, CRC risk was not significantly associated with female gender, young-aged or elderly, non-consumers of tobacco, alcohol consumption, and increased BMI (25–29.9 and 30–34.9). In HBV seropositive patients in Taiwan, it was shown that they had a higher risk (36% higher) of developing CRC, in comparison with seronegative (HBsAg negative) patients [ 53 ]. Moreover, the risk of synchronous colorectal liver metastasis (CRLM) was enhanced in cases with chronic HBV infection, demonstrating a dismal prognosis, in comparison with patients with occult hepatitis B infection (OBI) that exhibited a lower risk of metachronous CRLM and a longer free of disease post-surgical survival.…”

“…Chronic HBV causes approximately 60% of HCC worldwide, whereas 20% and 50% of cases with HCC in Asia and in the USA, respectively, are associated with chronic HCV. 17 24 Except for HCC, chronic hepatitis is also related to hematologic malignancies, including B cell non-Hodgkin's lymphoma (B-NHL) 25 and to some second primary solid malignancies including colorectal cancer, 26 intrahepatic cholangiocarcinoma, 25 head and neck, renal and pancreatic cancers. 25 27 28 In chronic hepatitis setting, any additional reconstitution of immune control after immunosuppressive medications could trigger viral reactivation.…”
