Correlation between posterior vaginal wall defects assessed by clinical examination and by defecography

Abstract: To estimate the accuracy of clinical examination and the indications for defecography in patients with primary posterior wall prolapse. Fifty-nine patients with primary pelvic organ prolapse were evaluated with a questionnaire, clinical examination and defecography. Defecography was used as reference standard. There was no relation between bowel complaints and posterior wall prolapse evaluated by clinical examination (p = 0.33), nor between bowel complaints and rectocele (p = 0.19) or enterocele (p = 0.99) ass… Show more

“…Finally, one may question whether POP-Q scores are the best representation of abnormalities of the rectovaginal wall [7, 30] since imaging techniques (defecography, MRI) can reveal anatomical abnormalities of the posterior compartment that are not represented by abnormal POP-Q scores [31]. …”

“…It is often used in women with posterior compartment prolapse and evacuation dysfunction. Groenendijk et al determined that defecography has a diagnostic accuracy of 42% for the posterior compartment with high sensitivity and low specificity for rectoceles compared to POP-Q [49]. Unfortunately, definitions of rectocele based on defecography vary widely with 48% of nulliparous asymptomatic women demonstrating rectocele on defecography [4].…”

“…Unfortunately, definitions of rectocele based on defecography vary widely with 48% of nulliparous asymptomatic women demonstrating rectocele on defecography [4]. Multiple studies have little to weak correlation between physical examination by POP-Q and defecography [49][50][51].…”
