Correlation of Modified Radioallergosorbent Test Scores and Skin Test Results

Abstract: In addition to a significantly increased sensitivity as compared with the initial Phadebas radioallergosorbent test, a major advantage of the Fadal-Nalebuff modified RAST is its correlation with skin testing using skin end point titration. This correlation allows physicians to use both these modalities in the diagnosis and treatment of allergic disorders. However, it has been anecdotally believed that the correlation of radioallergosorbent test classes and skin test end points varied somewhat with different an… Show more

“…As a simpler means of accomplishing this, serum fungal sIgG and sIgE can be measured by radioallergosorbent test (RAST). In vitro testing via modified RAST is an accepted standard in screening and diagnosing inhalant allergy, including molds 13–15 . Unfortunately, there have been no controlled studies to examine the diagnostic efficacy of these values for AFS.…”

“…24 In its Summary Statements of Practice Parameters for Allergy Diagnostic Tests, the American Academy of Allergy, Asthma, and Immunology (AAAAI) states it is impossible to quantify the exact amount of injected material by prick/puncture tests. 5 This statement may help to explain the findings of Tandy et al, 25 who did not find a good correlation with positive multiple skin prick device test results and modified RAST, whereas there was good correlation with comparative negative results. Despite these statements and findings, there are third-party payors whose only approved methods of skin testing for inhalant allergies are SPT and single intradermal tests.…”

“…Skin tests are more sensitive than in vitro specific IgE detection to identify sensitization to moulds and are considered the gold standard . However, in vitro tests as the modified RAST (mRAST) or the ImmunoCAP (Pharmacia Diagnostics, Piscataway, NJ) have shown good correlation with skin testing . Chamber et al.…”

“…Skin tests are more sensitive than in vitro specific IgE detection to identify sensitization to moulds and are considered the gold standard [59][60][61]. However, in vitro tests as the modified RAST (mRAST) [62] or the ImmunoCAP (Pharmacia Diagnostics, Piscataway, NJ) have shown good correlation with skin testing [59,60,63]. Chamber et al [59] compared both in vitro tests in the detection of allergy to Alternatia using skin testing as the gold standard and observed a specificity of 100% with both ImmunoCAP and mRAST; however, the sensitivity was 89% for ImmunoCAP and only 22% for mRAST.…”
