Correlation of serum albumin and prognostic nutritional index with outcomes following pancreaticoduodenectomy

Abstract: BACKGROUNDPancreaticoduodenectomy (PD) is a complex surgical procedure with a high morbidity rate. The serious complications are major risk factors for poor long-term surgical outcome. Studies have reported an association between early postoperative prognostic nutritional index (PNI) and prediction of severe complications after abdominal surgery. However, there have been no studies on the use of early postoperative PNI for predicting serious complications following PD.AIMTo analyze the risk factors and early p… Show more

“…The serum albumin concentration is affected by multiple factors, including nutritional status, liver failure, malignancy, surgical trauma, bacterial infection, inflammation, acute and chronic comorbidities (e.g., diabetes) and other conditions. 15 In the present study, all our patients experienced trauma. It is known that the stress response to injury is a factor that contributes to low preoperative serum levels.…”

“…17,25 Kamath A et al 20 reported that hypoalbuminaemia was independently associated with an increased risk of developing deep organ surgical-site infections, surgical-site infections, urinary traction infections, pneumonia and sepsis after revision total knee arthroplasty. Rungsakulkij N et al 15 reported that perioperative albumin was an important factor associated with serious complications following pancreaticoduodenectomy, include POP. This evidence was also supported by Munteanu A et al, 26 who reported that serum albumin levels, checked both before and after surgery, were of great value in helping predict a series of postoperative complications in gastric cancer surgery.…”

“…The factors identified in previous studies as risk factors for SSI after PD include preoperative biliary drainage [2,7], pancreatic fistula [8], a high body mass index (BMI) [8], diabetes mellitus [7], blood transfusion [9], malnutrition, and a low serum albumin level [10][11][12]. As previous studies have revealed varying results, the primary aim of the present study was to identify the factors associated with SSI after PD, and the secondary aim was to identify adverse outcomes related to SSI after PD.…”

“…The preoperative serum albumin level is strongly associated with outcomes following various types of gastrointestinal surgery, including gastric, pancreatic, and colorectal surgery , as well as non‐gastrointestinal surgery . The association between serum albumin and IAI is controversial .…”
